Antitüberküloz tedaviye bağlı geç oluşan Addison olgusu

Özet

Addison hastalığı, sürrenal bezin otoimmün nedenli veya enfeksiyon hastalıklarına bağlı olarak gelişebilen hipofonksiyonudur. Tüberküloz tedavisi sırasında Rifampisinin mikrozomal enzim indüksiyonu aktivitesine bağlı olarak gelişebildiği bildirilmiştir. Tüberküloz tanısı konulan olgularda antitüberküloz tedavi bildirilmiştir. Ancak tedavinin geç döneminde Addison hastalığı gelişen olguya rastlanmamıştır. Dokuz ay önce halsizlik, 20 kg kaybı, yutma güçlüğü yakınmaları ile polikliniğimize başvuran kırksekiz yaşında bayan hastanın; gastroenteroloji polikliniğindeki klinik değerlendrimesi ve endoskopisinde patolojik bulgu saptanmayan hastanın özgeçmişinde tüberküloz temas öyküsü yoktu. Bilinen ek hastalığı yoktu. Sistem sorgulamasında öksürük, balgam, hemoptizi, nefes darlığı, göğüs ağrısı, ateş ve gece terlemesi tanımlamıyordu. göğüs muayenesi doğaldı. Hepatosplenomegali, periferik değerlerinde hemogram, sedimentasyon ve C-reaktif protein düzeyleri normaldi. PPD: 20 mm, serum anjiokonverting enzim düzeyi: 30 mg/dl (8-52 mg/dl) idi. PA akciğer grafisinde patolojik bulgu saptanmadı. Toraks bilgisayarlı tomografisinde subkarinal lenf nodu dışında patolojik bulgu yoktu. Hastaya yapılan bronkoskopide endobronşial lezyon yoktu. Bronş lavajı asidorezistanbasil (ARB), mikobakteri polimeraz zincir reaksiyonu (PCR) negatifti. Endobronşial ultrasonografide (EBUS) 35 mm'lik, hipoekojen, içinde kalsifikayon bulunan lenf nodundan 5 kez EBUS-Transbronşial iğne aspirasyonu (TBİA) yapıldı. Transbronşial iğne aspirasyon yapıldı. Sonucunun kazeifikasyon içeren granülom şeklinde gelmesi üzerine, hasta mediastinal tüberküloz lenfadenit tanısı almıştır. Takiplerinde antitüberküloz tedavi alırken, radyolojik düzelme izlendi. Ancak hastanın tedavinin 8. Ayında aynı semptomlarla başvurması üzerine yapılan fizik muayenede hiperpigmentasyon varlığının gözlenmesi üzerine, endokrinolojik olarak değerlendirilmiş ve Addison hastalığı tanısı konmuştur. Hastaya kortikosteroid replasman tedavisi başlanmış ve klinik düzelme izlenmiştir. Olgumuz, mediastinal tüberküloz tanısı ile antitüberküloz tedavisi alırken tedavisinin 8. ayında sürrenal yetmezlik tablosu gelişmesi nedeni ile sunulmuştur.

Abstract

Addison's Disease that evolved due to infection orcaused by autoimmune disease is a hypofunction ofthe adrenal gland. Addisson's Disease might developdue to microsomal enzyme induction associated withtreatment with Rifampisin. It is noted that the casereports of those who were diagnosed with Addison'sDisease after the antituberculosis treatment hadbeen started. However, there are no cases whereAddison's Disease developed in the late periodof antituberculosis treatment in the literature.A fourty eight year old female patient applied toour policlinic with complaints of weakness, twentykg weight loss and difficulty in swallowing ninemonths ago. There was no history of tuberculosis inthe patients background. It wasn't determined frompathological sign on her examination and endoscopyin the policlinic of Gastroenterology. She didn'nhave comorbid disease. Cough, sputum, hemoptysy,dyspnoe, chest pain, fever and night sweating werenot in her complaints. Chest examination werenormal. There was no hepatosplenomegaly. Serumhemogram, sedimentation and C-reactive proteinwere normal. PPD: 20 mm, Serum angioconverting enzym value: 30 mg/dl (8-52 mg/dl). Posteroanteriorchest CAT scan was normal. There were not any othersigns except subcarinal lymph nodes which are seenon thorax computarised tomograpy. Endobronchiallesion was not on her bronchoscopy. Asidoresistanbasil(ARB), Mycobcteria Polymerase Chain Reaction (PCR)was negative in her bronchial lavage. Transbronchialneedle aspiration (TBNA) awas taken from asubcranial, hypoechoic, calcified lymph node of 35mm using Endobronchial Ultrasound (EBUS). Patientwas diagnosed with mediastinal tuberculosis due togranuloma containing calcification by pathologiston her biopsy. While she was taking antituberculosistreatment, her radiological signs were improved infollow up. However, she with the same complaintson the eighth month of treatment. Because ofhyperpigmentatiton seen on her examination, shewas evaulated by endocrinologist and diagnosed withAddisson's Disease. Corticosteroid treatment was tothe patient and clinical improvement was achieved.This case was reported because while she was takingantituberculosis treatment on the eighth month,Addison's Disease had developed in the patient.
