Anjioimmünoblastik T-hücreli lenfoması olan bir hastada çoklu ilaç dirençli Corynebacterium mucifaciens'in neden olduğu ölümcül bir sepsis olgusu

Özet

Amaç: Son yıllarda Corynebacterium türlerifırsatçı patojenler olarak dikkat çekmeyebaşlamıştır. Corynebacterium mucifaciens deri, kanve diğer steril vücut sıvılarından izole edilmiştir.1997'de tarif edildiğinden bu yana nadireninsan patojeni olarak bildirilmiştir. Bu yazıdafebril nötropeni ve septik şok ön tanısı ile İ.Ü.Cerrahpaşa Tıp Fakültesi Sadi Sun Yoğun BakımÜnitesinde takibe alınan anjioimmünoblastikT-hücreli lenfoma tanısı almış 58 yaşında birerkek olgu sunulmaktadır. Kan kültüründeüreyen Gram pozitif çomak şeklinde bakterilerinidentifikasyonu Phoenix (BD Diagnostic Systems,Sparks, MD) otomatize idendifikasyon sistemikullanılarak yapılmıştır ve 16S rRNA dizi analiziile doğrulanmıştır. Bakterinin antibiyotik MIKdeğerleri E test ile saptanmıştır ve EUCASTkriterlerine göre değerlendirilmiştir. Hastadanalınan 4 kan kültür örneğinde saf olarakGram pozitif çomak şeklinde üreyen bakteri,C. mucifaciens olarak tanımlanmıştır. Bakterikökeninden elde edilen 16S rRNA dizisi,GenBank veri bankası ile yapılan karşılaştırmadaC. mucifaciens ile uyumlu bulunmuştur. BakterininE test ile yapılan antibiyotik MIK değerleriseftazidim, klindamisin, azitromisin ve klaritromisiniçin >256 µg/mL, sefotaksim ve siprofloksasin için32 µg/mL, penisilin, ampisilin ve sefepim için 16 µg/ mL, imipenem için 6 µg/mL, piperasillin+tazobaktamiçin 8 µg/mL, amoksisilin+klavulanik asit için 3µg/mL, tobramisin için 0,16 µg/mL, amikasiniçin 1,25 µg/mL ve teikoplanin için 2 µg/mLolarak tespit edilmiştir. Vankomisin (MİK, 1 µg/mL)ve gentamisin (MİK, 0,16 µg/mL) C. mucifaciens'eetkili ilaçlar olarak bulunmuştur. Hasta yapılantüm müdahelelere rağmen septik şok ve çokluorgan yetersizliği nedeniyle kaybedilmiştir.C. mucifaciens immün-yetmezlik öyküsü olanhastalarda önemli bir insan patojeni olarakdüşünülmelidir.

Abstract

Objective: In recent years, Corynebacteriumspecies has attracted attention as opportunisticpathogens. Corynebacterium mucifaciens has beenisolated from skin, blood and other sterile body fluids.It has been rarely reported as human pathogens,since it was described in 1997. In this study, a 58years old male patient who has a medical historyof angioimmunoblastic T-cell lymphoma, taken tothe Sadi Sun Intensive Care Unit in I.U. CerrahpasaMedical Faculty Hospital with the preliminarydiagnosis of febrile neutropenia and septic shockwas reported. The identification of Gram-positiverod-shaped bacteria isolated in blood cultures wasperformed by using the Phoenix (BD DiagnosticSystems, Sparks, MD) automated identification systemand was confirmed by 16S rRNA sequence analysis.Bacterial antibiotic MIC values were determined byE-test and evaluated according to EUCAST criteria.Gram-positive rod-shaped bacteria which wereisolated from 4 blood culture samples taken fromthe patient, were identified as C. mucifaciens.The 16S rRNA gene sequence which obtained fromthe bacteria was matched with C. mucifacienssequences in GenBank database. AntibioticMIC values with E test were determined as;>256 µg/mL for ceftazidime, clindamycin,azithromycin, and clarithromycin, 32 µg/mLfor cefotaxime and ciprofloxacin, 16 µg/mL for penicillin, ampicillin and cefepime, 6 µg/mL forimipenem, 8 µg/mL for piperacillin + tazobactam,3 µg/mL for amoxicillin + clavulanic acid, 0.16 µg/mL for tobramycin, 1.25 µg/mL for amikacin and2 µg/mL for teicoplanin, respectively. Vancomycin(MIC, 1 µg/mL) and gentamycin (MIC, 0.16 µg/mL)were found to be the effective antibiotics againstC. mucifaciens. Despite all of the interventionsperformed patient passed away due to septic shockand multiple organ failure. C. mucifaciens shouldbe considered as an important human pathogen inpatients with history of immunodeficiency.
