Özet
Amaç: Kan akımı enfeksiyonları (KAİ) mortaliteyi ve morbiditeyi arttıran önemli hastane enfeksiyonlarından biridir. KAİ'nin erken tanısı, infeksiyona neden olan organizmanın tespit edilmesi ve antimikrobiyal duyarlılık testlerinin yapılması hastanın prognozu açısından oldukça önemlidir. Bu çalışma da kan kültürlerinden izole edilen mikroorganizmalar ve antimikrobiyal duyarlılıkları etkenlerinin dağılımı ve antimikrobiyal ilaç duyarlılığının ortaya konması amaçlandı.incelenerek, hastanemizdeki KAİ kan kültürü örneklerinin 697 (% 23,9)'sinde üreme oldu, 89 (%3,04)'u kontaminasyon olarak değerlendirilirken, 2137 (%73,1)'sinde üreme tespit edilmedi. Üreyen izolatlardan 113 (%16,2) tanesi mükerrer izolat kabul edilip değerlendirme dışı bırakılarak 584 (%20,0) izolat değerlendirmeye alındı. Değerlendirmemiz sonucunda; patojenler arasında ilk sırayı %58,2 ile koogüloz negatif stafilokok (KNS) ve bunu %8 ile Escherichia coli, %7,9 ile Acinetobacter spp. aldığı izlendi. Bakterilerin antibiyotik duyarlılıklarına bakıldığında, KNS'lerde metisiline direncin %54,9, Staphylococcus aureus'da %34,4 olduğu tespit edildi. Buna karşılık vankomisin ve linezolide karşı iki bakteri türünde de direnç saptanmadı. Enterokoklarda ise %55,6 penisilin direnci belirlendi. Bir (%4,5) hastada vankomisin direnci saptanırken, linezolid ve teikoplanin direnci tigesikline duyarlı olduğu görüldü. Klebsiella spp.'de %5,9 oranında imipenem direnci saptandı. Acinetobacter spp'nin en çok tigesikline (%2,4) duyarlı olduğu görüldü. görülmedi. Enterobacteriaceae ailesinin Sonuç: Klinisyenlere yol göstermesi açısından ampirik tedavi protokollerinin güncellenmesi, doğru antibiyotik kullanımı için belirli zaman aralıklarında kan kültürlerinden izole edilen mikroorganizmaların dağılımını ve duyarlılık paternini gösteren çalışmaların yapılması gerekmektedir.
Abstract
Objective: Blood stream infections (BSI) is oneof the significant hospital-acquired infections thatincrease mortality and morbidity. Early diagnosis of BSI,identification of microrganisms that cause infection andanalyzing antimicrobial sensitivity tests are important interms of patient's prognosis. In this study microorganismsisolated from blood cultures and their antimicrobialsensitivity were investigated. Besides, to reveal thedistribution of our hospital's BSI pathogens and to presenttheir antimicrobial sensitivity paterns, were aimed.Methods: In this study blood culture samplescollected at Kahramanmaraş Necip Fazıl City Hospitalbetween September 2012 - May 2014, were examined.Samples were incubated with BACTEC/90050 (BectonDickinson, Maryland, the USA) automatization system.For the identification of microorganisms, Vitek version2.0 (Biomerieux, France) automatization systemwas used in addition to conventional methods whennecessary. Antibiotic sensitivity tests were studied incompliance with Clinical and Laboratory StandardsInstitute (CLSI) standards with Kirby-Bauer disc diffusionsusceptibility test.Results: During our study period 2.923 bloodcultures were analysed. Six hundred ninety seven (23,7%) samples gave reproductive signal as positive. Weobserved contamination in 89 (12.8%) of samples. In 2137(73.1%) samples reproductive signal was not received.While repeating isolates were excluded from study, 584(20.0%) isolates were included in the study. The mostcommonorganisms causing BSIs were coagulase negativestaphylococci (CNS) (58.2%), Escherichia coli (8%)and Acinetobacter spp. (7.9%). Methicillin resistancewas detected in 54.9% of CNS isolates and in 34.4%Staphylococus aureus isolates. However vancomycin andlinezolid resistance were not detected in both of thebacteria. For Enterococci, 55.6% penicilline resistancewas determined. For one patient (4.5%) vancomycineresistance was detected, linezolid and teichoplaminresistance were not stated. Isolates belonging toEnterobacteriaceae family were sensitive to tigecycline.Among Klebsiella spp., 5.9% of the isolates were resistantto imipenem. 2.4% of Acinetobacter spp. isolates wereresistant to tigecycline.Conclusion: To refer clinicians, there is need tomake studies about distribution of microorganisms andtheir antibiotic sensitivity patterns which are isolatedfrom blood cultures in certain time intervals forupdating empirical treatment protocols and right usageof antibiotics.
