Pediatrik yaş grubunda kemoterapiye bağlı kusmalarda tropisetronun etkinliği

Özet

Bulantı ve kusma kemoterapiye bağlı en sık görülen ciddi yan etkilerdir ve konvansiyonel antiemetik-lerle kontrol altına alınamamaktadır. Bu çalışmada bir 5HT3 reseptör antagonist olan tropisetronun klinik uygulamalardaki etkinliği araştırıldı. Tropisetron yaşlan 2 ile 14 arasında (ortalama 10.8 yıl), 8'i kız (%30.8), 18'i erkek (%69.2) 26 maligniteli hastada, toplam 40 kemoterapi uygulamasında veril-di. Vücut ağırlığı 10 kilogramın altındakilere 0.2 mg/kg/gün, 10 kilogramın üzerindekilere 5 mg/m2/gün maksimum 5 mg dozunda, kemoterapiden 30 dakika önce intravenöz (TV), daha sonra 24 saat aralıklarla aynı dozlarda 4 gün süre ile ağızdan (PO) uygulandı. Olgulara her bir kemoterapi uygulamasında yüksek doz metotreksat (>1 g/m2), yüksek doz ifosfamid (>800 mg/m2), yüksek doz siklofosfamid (>800 mg/m2) veya sisplatin tek başına veya diğer kemoterapötiklerle birlikte uygulandı. Bulantı ve kusma otuzdokuz kemoterapi uygulamasında (%97.5) kontrol edildi. Tropisetron uygula-masına bağlı yan etki saptanmadı. Mevcut bulgular tropisetronun yüksek emetojenik kemoterapi alan pediatrik maligniteli hastalarda etkin ve güvenilir olduğunu göstermektedir.

Abstract

Nausea and vomiting are common side effects in children receiving anticancer chemotherapy and are not controlled with conventional antiemetics. We examined the effectiveness of tropisetron, a 5HT3 receptor antagonist, in clinical usage. Twenty six malignant patients, 8 girls (30.8%), 18 boys (69.2%), aged 2-14 years (mean 10.8 year) received tropisetron in 40 chemotherapy cycles. Tropisetron was administered 0.2 mg/kg/dayfor body weight under 10 kg, 5 mg/m2 for body weight over 10 kg (maxi-mum 5 mg) 30 minutes before chemotherapy via IV route. Doses were repeated with intervals of 24 hours for 4 days orally. Methotrexate (> 1 g/m2), ifosfamid (> 800 mg/m2), cyclophosphamide (> 800 mg/m2) or cisplatin were administred single or with the other anticancer agents. Nausea and vomiting were con-trolled in 39 chemotherapy cycles (97.5%). No side effects were detected during tropisetron therapy. In conclusion, the results showed that tropisetron is effective and reliable in children receiving high eme-togenic chemotherapy regimens.
