Özet

Yüksek dozda kemoradyoterapi ve kemik iliği transplantasyonunu (KiT) takip eden dönemlerde şiddet-li hemorajik ve trombotik değişiklikler oluşabilir. Hematolojik ve solid malignitelere sahip olan ve alloje-neik KiT (7 olgu) ve otolog periferik kan kök hücre transplantasyonu (2 olgu) uygulanan hastalarda yapılan bu çalışmada hemostatik parametreleri takip ettik. Trombosit sayımı, protrombin za'manı, par-siyel tromboplastin zamanı, FVJII : C, plazma fibrinojen düzeyi, kanama zamanı, ve ADP, kollagen, epinefrin, düşük ve yüksek doz ristosetinin indüklediği trombosit agregasyonu hazırlama rejiminden önce, ve -l, +1, +15, +90. günlerde çalışıldı. Trombositopeni ve buna bağlı olarak agregasyon değerle-rinde azalma ile KiT öncesi ve sonrasında hemorajik bir eğilim tespit edildi. Protrombin zamanlarında da çalışma süresince anlamlı değişiklikler gözlendi. Ayrıca, KiT'i takiben gözlenen fibrinojendeki prog-ressif artış, protrombotik duruma katkıda bulunan bir faktör olabilir. Sonuç olarak, bu çalışmada KiT hastalarında hemorajik bir eğilim tespit edilmiştir. KiT hastalarında sık ve şiddetli olarak gözlenen he-mostatik değişikliklerin anlaşılmasında hemorajik ve trombotik durumun daha detaylı laboratuvar tet-kikleriyle değerlendirilmesi uygun olacaktır.

Abstract

Severe hemorrhagic and thrombotic changes may occur in the period following high-dose chemoradiot-herapy and bone marrow transplantation (BMT). In this study, performed in patients with hematologi-cal and solid malignancies subjected to allogeneic bone marrow transplantation (7 cases) and autolo-gous peripheral stem cell transplantation (2 cases), we have monitored laboratory hemostatic parame-ters. Platelet count, prothrombin time, partial thromboplastin time, FVIII : C, plasma fibrinogen, ble-eding time and ADP, collagen, epinephrin, low and high dose ristocetin-induc'ed platelet aggregation studies were assessed before conditioning therapy and on day -1, +1, +15, +90 thereafter. A hemorrha-gic tendency -with thrombocytopenia and reduced values of platelet aggregation were documented befo-re and after transplantation. During the study period, significant changes were observed in prothrom-bin time values as well. Furthermore, after transplantation, progressive increase of plasma fibrinogen may be a contributing f actor to the prothrombotic state. In conclusion, in this study a state ofhemorr-hagic tendency was documented in BMT'patients. It is suggested that a more detailed laboratory evalu-ation of hemorrhagic and thrombotic state should be performed to clarify the frequent and severe he-mostatic changes observed in the BMT patients.
