Akut lösemilerde Ki-67 içeriği ile remisyon oranlarının karşılaştırılması

Özet
Ki-67 proliferasyon fazında olan hücrelerin çekirdeğinde bulunan bir proteini tanıyan monoklonal antikordur. Yüksek Ki-67 taşıyan solid tümörlerde prognozun kötü olduğu bilinmektedir. Akut lösemilerde ise proliferasyon fazında blastların bazı kemoterapötik ilaçlara daha hassas olduğu ve remisyona daha kolay girdikleri ileri sürülmüştür. Bu çalışmada, akut lösemi blastlarının Ki-67 içeriğini tespit edip, hastaların remisyon indüksiyon tedavisine verdikleri cevapları karşılaştırıldı. Otuz bir akut lösemili hastanın blastları Ki-67 saf monoklonal antikor kullanılarak immünhistokimyasal yöntem ile boyandı. AML hastaları 3+7, ALL hastaları BFM-84 protokolüne göre tedavi edildi. On beş hasta remisyona girdi, bir hasta tedavi öncesi kaybedildi. Ortanca Ki-67 (+) hücre yüzdesi remisyona giren hastalarda 32 (1-70), remisyona girmeyen hastalarda ise 20 (1-52) olarak bulundu. Fark istatistiksel olarak anlamlı değildi. Bulgularımız Ki-67 (+) liğinin remisyon indüksiyon tedavisine bir etkisinin olmadığını düşündürmektedir, ancak prognostik öneminin değerlendirilebilmesi için çok sayıda hasta içeren, uzun süreli ve çok değişkenli analizlerin yapılması gerekmektedir.
Abstract
Ki-67 is a monoclonal antibody, which recognizes a protein present in the nucleus of proliferating cells. Although high Ki-67 expression has been known to be a poor prognostic marker in solid tumors, it was suggested that to be in proliferation phase, the blast cells would be sensitive to cell-cycle-specific drugs in acute leukemias. The aim of our study was to determine the expression of Ki-67 on acute leukemia blast cells and to evaluate their effect on remission induction therapy. Thirty one acute leukemia patients were included into the study and, their blast cell smears were stained immunohistochemically with Ki-67 pure monoclonal antibody. AML patients were treated with 3+7 protocol and BFM-84 were used to treat ALL patients. Remission was attained in 17 patients, one patient died before therapy. The medyan of Ki-67 positive cells was 32 % (range 1-70) in patients who attained remission, 20 % (range 1-50) in patients who did not and, the difference was not found to be significant (p>0.05). Our results indicated that Ki-67 percentage of blast cells were not involved in remission induction therapy. Long term analysis with large patient groups and with multivariate analysis should be performed in order to define prognostic significance of Ki-67 in acute leukemias .
