Özet

Erişkin akut nonlenfoblastik lösemide (ANLL) immünofenotiplendirmenin klinik önemini tanımlamak amacıyla 18 ANLL'li hasta (8 bayan, 10 erkek) prospektif olarak incelendi. Bu amaçla, myeloblastlarda tedaviden önce myeloid antijenlere, B-hücrelerine ve T-hücrelerine karşı monoklonal antikorlarla reaktivite kontrol edildi. Vakalardan 13'ünde aberran lenfoid antijen ekspresyonu vardı. Bunlardan 7'si B-hücre antijeni taşımaktaydı Yedi hastanın 6'sında CD19 ekspresyonu ve 1'inde de CD19 ve CD22 birlikteliği vardı. Bu 7 vakadan 5'i ayrıca CD34 ekspresyonu göstermekte idiler. T-hücre antijeni taşıyan 3 vakadan 2'si CD2 ve CD34 diğeri ise CD4 taşımaktaydı. Geriye kalan 3 vakanın 2'sinde CD19 ve CD7 birlikteliği, 1'inde CD19, CD22 ve CD7 birlikteliği vardı. Bu çalışmada ANLL blastlarının myeloid antijen ile birlikte lenfoid antijen ekspresyonunun prognostik önemi saptanmadı.

Abstract

Eigh teen patients (8 female, 10 male) with acute nonlymphoblastic leukemia (ANLL) were investigated prospectively for determining the clinical significance of immunophenotyping in adult ANLL. For this purpose, reactivity with monoclonal antibody in myeloblasts against myeloid antigens, B-cells and T-cells was determined in the pretreatment period. Aberrant lymphoid antigen expression was found in thirteen patients with ANLL. Seven of 13 patients expressed B-cell antigen. Six of 7 patients expressed CD19 and one patient had CD19 and CD22 coexpression. Five of these 7 patients had also CD 34 expression. Two of 3 patients who expressed T-cell antigen had CD2, CD34, whereas the other patient had CD4 antigen. CD19 and CD7 coexpression in 2 of last 3 patients and CD19, CD22 and CD7 coexpression in one patient were found. In this study suggests that expression of lymphoid-associated cell surface antigens as well as myeloid-associated antigens by ANLL blasts lacks prognostic significance.
