Survival and prognostic factors in patients with Hodgkin's disease

Özet
Bu çalışmada kliniğimizde tedavi ve izlemi yapılan Hodgkin lenfomalı hastalarda sağkalım ve prognostik faktörler araştırıldı. HASTALAR VE METOD: Bu çalışmada 1991-98 yılları arasında Hodgkin hastalığı tanısıyla kliniğimizde tedavi ve izlemi yapılan 125 hasta retrospektif olarak incelendi. Hastalar erken evre-düşük risk (I-IIIa, group 1) ; erken evre-yüksek risk (group 2) ve ileri evre (IIIb-IV; group 3) olmak üzere 3 prognostik gruba ayrılarak kliniğimizin protokoluna göre tedavi edildiler. Yüksek riskli hastaların seçiminde kullanılan major kötü prognoz faktörleri; lenfositten fakir histoloji, mediasten/toraks oranı>1/3, B semptomları ve nodal bölge>3; minör kötü prognoz faktörleri ise yaş>50 , ESH (eritrosit sedimentasyon hızı) >40 mm/st., mikst hücreli histoloji ve tümör çapı>6 cm. idi. SONUÇLAR: Erkek/kadın oranı: 85/40, medyan yaş ortalaması 34 (15-84) olarak bulundu. Histolojik tiplemede hastaların 17'si (%12.8) lenfositten zengin; 30'u (%24) nodüler sklerozan; 64'ü (%51.2) mikst hücreli, 14'ü lenfositten fakir (%11.2) olarak bulundu ; 1 hasta (%0.8) sınıflandırılamadı. Tüm grupta 87 hastada (%69.6) kötü prognoz faktörleri vardı. Erken evre (evre I, II, IIIa) ve kötü prognoz faktörü olmayan hastalara (n:37,%29.6) sadece radyoterapi uygulandı. Evre IIIb ve IV hastalar (n:27, %21.6) kemoterapi ve gerekirse radyoterapi ile; erken evre olup kötü prognoz faktörü olan hastalar (n:61,%48.8), kemoterapi ve peşisıra radyoterapi ile tedavi edildiler. Tüm grupta medyan 40.0 aylık takip süresince 70 hasta (%55.6) hastalıksız olarak hayattadır. Erken evre olan ve kötü prognoz faktörü bulunmayan grupta genel sağkalım süresi ortalama 90.51 ay, 7 yıllık sağkalım oranı % 97.0 olarak izlendi. Erken evre ve kötü prognoz faktörü (+) olan hastalarda ise ortalama sağkalım 75.54 ay, 5 ve 7 yıllık sağkalım oranları sırasıyla % 81.04 ve %60.06 olarak bulundu. Evre IIIb ve IV hastalarda genel sağkalım ortalama 77.12 ay, 3 yıllık sağkalım oranı % 80.84; tüm hastalar için , ortalama sağkalım 82.62 ay , 5 ve 7 yıllık genel sağkalım oranları sırasıyla %86.49 ve %76.94 olarak saptandı.Grupların hiçbirinde medyan sağkalım süresine ulaşılamadı. Bütün hasta grubunda tek değişkenli analizde kötü prognoz faktörleri (p:0.0089) dışında, ESH >40 mm/st. (p:0.0004) ve yaş >50 (p:0.0001) genel sağkalım üzerinde olumsuz etkileri olan bağımsız prognostik faktörler olarak bulundu. Evre IIIb ve IV hastalarda ise mediastinal tutulum sağkalım üzerinde etkisi olan tek prognostik faktör olarak gözlendi (p:0.02). Tüm grupta progresyona dek geçen süre medyan 34.0 ay; ortalama progresyonsuz sağkalım 73.37 ay; 5 yıllık progresyonsuz sağkalım oranı %74.14 olarak izlendi. Tek değişkenli analiz sonucu kötü prognoz faktörleri (p:0.012); ESH $geq$40 mm/st. (p:0.01); yaş $geq$50 (p:0.047) ve lenfositten fakir histoloji (p:0.036) ile progresyonsuz sağkalım arasında anlamlı bir ilişki saptandı. TARTIŞMA: Sonuç olarak, ESH (40 mm/st.) ve yaş $geq$50 faktörlerinin majör kötü prognostik faktörler olarak kabul edilerek tedavi seçiminin buna göre belirlenmesinin yararı olacağı düşüncesindeyiz.
Abstract
The aim of this study is to determine the overall and progression free survival in patients with Hodgkin Disease (HD) and evaluate the prognostic factors associated with an adverse outcome. Methods: Patients with histologically confirmed HD; treated and followed in our clinic between 1991-98 were evaluated retrospectively for response to treatment, survival and prognostic factors. Patients were classified into 3 prognostic groups: early stage (I-IIIa) without clinical prognostic factors (group 1), early stage with clinical prognostic factors (group 2), advanced stage patients (IIIb-IV; group 3) and treated accordingly. Results: Out of 125 patients, 27(21.6%) patients with advanced disease, 56(44.8%) with early stage and additional clinical prognostic factors were treated with primary chemotherapy. Seventy-one patients (85.6%) were given standard anthracycline-based combination. After a median follow-up period of 40 months; 70 (55.6%) patients remain with no evidence of disease and 16 (19.3%) have died. Overall survival (OS) at 7 years in the 1st group was 97.0%; in the 2nd group OS at 5 and 7 years were 81.0% and 60.0%, respectively and that of the 3rd group at 3 years was 80.8%.OS at 5 and 7 years for the whole group were 86.5 and 76.9%, respectively. Progression free survival (PSF) for the latter group at 5 years was 74.1%.Median survival was not reached in either group.Univariate analysis revealed that ESR (40mm/hr) (p:0.0004), age ³ 50 years (p:0.0001) and the presence of clinical prognostic factors (p:0.0089) were associated with a poor prognosis.Age $geq$50 years and ESR $geq$40mm/hr were shown to be independent prognostic factors by multivariate analysis. Conclusion: ESR(40mm/hr) and age $geq$50 years are major prognostic factors with an adverse effect on the outcome of patients with HD.
