Osteosarkom ve mezankimal kondrosarkomlu olgularımızın değerlendirilmesi

Özet

Eylül 1989-şubat 1999 tarihleri arasında İstanbul Üniversitesi Onkoloji Enstitüsü Pediatrik Onkoloji Bilim Dalına 53, Kartal Eğitim ve Araştırma Hastanesine 4 olmak üzere toplam 57 Osteosarkom ve Mezankimal Kondrosarkomlu çocuk başvurmuş olup, bunların 49'u Osteosarkom, 8'i Mezankimal Kondrosarkom tanısı almıştır. Yaşları 5-16 (median 13) yaş arasında değişen bu hastaların 33'üerkek, 24'ü kızdır (E/K=1,4). Bölümümüzde primer olarak tedavi gören 57 olguda (47 osteosarkom, 2 küçük hücreli osteosarkom, 8 mezankimal kondrosarkom) tanıdaki tümör lokalizasyonu 25 (%44) olguda femur, 13(%23) olguda tibia, 5(%9) olguda fibula, 4 (%7) olguda humerus, 2'şer(%4) olguda vertebra, pubis, ulna ve çene, 1'er (%2) olguda ise radius ve temporo-parietal kemiktir. Kemoterapi protokolü olarak 3 kür cerrahi öncesi, 3 kür cerrahi sonrası olmak üzere toplam 6 kür kemoterapi (Epirubisin 90 mg/m2, 1.nci gün + Ifosfamid 1,8g/m2 X 3 gün + Sisplatin 100 mg/m2, 4. gün) uygulandı. Toplam 57 olgunun 10'u (%17) metastatik, 47'si (%83) nonmetastatik idi. Kliniğimizde toplam 57 olgunun 33'ünde (%58) cerrahi olarak rezeksiyon, 6'sında (%10) amputasyon yapıldı; 4 (%7) olgu ampute olarak başvurdu; 14 (%25) olguda ise çeşitli nedenlerle cerrahi yapılamadı. Ağrı palyasyonu veya cerrahi sınır müspetliği gibi nedenlerle 57 olgunun 14'ünde (%25) 1000-5000cGy arasynda radyoterapi uygulandı. Hastalar median 18(1-100) ay izlenmiş olup, 49 osteosarkom olgusunda, 3 yıllık genel sağkalım %60, olaysız sağkalım %46 idi.

Abstract

Between September 1989 and February 1999, 57 patients with osteosarcoma or mesenchymal chondrosarcoma of bone were treated in the University of İstanbul, Institute of Oncology. These patients (33 male, 24 female), aged between 5-16 years (median 13), consisted of 49 osteosarcomas and 8 mesenchymal chondrosarcomas. The location of the tumor at presentation was femur in 25 (%44), tibia in 13 (%23), fibula in 5 (%9), humerus in 4 (%7), vertebra, pubis, ulna and mandibula in 2 each (%4), radius and temporo-parietal in 1 each (%2). Patients received three courses of chemotherapy before and three after surgery that consisted of epirubicin (90 mg/m2 day1), ifosfamide (1,8 g/m2 daily for three consecutive days) and cisplatin (100 mg/m2 day 4). Of 57 patients, 10 (%17) were metastatic at diagnosis and 47 (%83) were localised. Thirty-three of them (%58) had limb-salvage surgery, 6 (%10) amputation after neo-adjuvant chemotherapy, 4 (%7) amputation at presentation. Fourteen didnÕt undergo surgery for various reasons. Radiotherapy ranging 1000-5000 cGy was given to 14 patients for palliating pain or the positive surgical margins. Three year overall and event free survival were 60% and 46% respectively with a median follow-up of 18 months (1-100 months).
