Ewing sarkom ailesi tümörlü çocukların değerlendirilmesi

Özet
Çalışmamızda Ewing sarkom ailesi tümörü tanısı alan çocukların demografik özellikleri, başlangıç tedavisine yanıt, sağkalım ve prognostik faktörler yönünden incelenmesi amaçlanmıştır. Eylül 1989-Mayıs 2000 tarihleri arasında medyan yaşı 12 (3-16) yaş olan 73 çocuğun (36 kız, 37 erkek) değerlendiril-mesi yapılmıştır. Başvuruda 18 çocukta metastatik, 55'inde lokalize hastalık saptanmıştır. Primer yerleşim 30 olguda ekstremite, 43 olguda ekstremite dışı idi. Hastalık 51 olguda kemik, 22 olguda yumuşak dokudan kaynaklanmaktaydı. Tedavi olarak 3 kür neoadjuvant IE/VAC (ifosfamid, etoposid, vinkristin, adriamisin/ aktinomisin D, siklofosfamid) kemoterapiyi takiben 9. haftada lokal tedavi (radyoterapi ve/veya cerrahi) uygulandı, kemoterapi 1 yıla tamamlandı. Üç kür başlangıç kemoterapisine yanıt oranı %80.5 idi. Lokal tedavi 34 hastada radyoterapi, 8 hastada cerrahi, 21 hastada radyoterapi + cerrahi şeklinde uygulandı. On dokuz olguda, medyan 17 ayda (4-41 ay), nüks veya progresyon gelişmiştir. Bunların 6'smda lokal nüks/progresyon, 12'sinde uzak metastaz, l hastada lokal nüks + uzak metastaz gözlendi. Tüm grubun 2 ve 5 yıllık sağkalım hızları %65 ve %51 idi. Tanıda metastatik hastalık (5 yıllık sağkalım %22 vs %63, p=0.02) olması ve başlangıç tedavisine yetersiz yanıt (5 yıllık sağkalım %54 vs 2 yıllık %20, p=0.01) prognozu kötü yönde etkilemekteydi. Yaş, cinsiyet, primer tümörün yerleşimi, kemik-yumuşak doku kaynaklı oluşu ve tümör boyutu prognozu etkilememiştir. Sonuç olarak serimizde Ewing sarkom ailesi tümörlerinde sağkalım diğer serilere uygunluk göstermektedir. Tanıda metastaz varlığı ve başlangıç tedavisine yanıt en önemli prognostik faktörler olarak bulunmuştur
Abstract
The aim of this study is to review Ewing sarcoma family tumors (ESFT) in regard to demographic features, response to initial chemotherapy, survival and prognostic factors. Seventy-three children (F;M=36;37) with a median age of 12 years (3-16) were admitted between September 1989 and May 2000. There were 55 localized and 18 metastatic patients. Primary localization was in the extremities in 30 and other sites in 43 patients. Fifty one patients. were osseous and 22 extraosseous. Initial treatment consisted of 3 cycles of IE/VAC (ifosfamide, etoposide, vincristine, adriamycine/actinomycin D, cyclophosphamide) chemotherapy; followed by local therapy [radiotherapy (RT) ± surgery (S)] at week 9; and adjuvant chemotherapy for a total of l year. The objective clinical and radiological response rate to three courses of initial chemotherapy was 80.5%. RT was performed for local control in 34, S in 8, RT + S in 21 patients. Nineteen patients had progressive disease or relapse (6 local, 12 distant, l local + distant) at a median time of 17 (4-41) months. The overall survival for the whole group at 2 years and 5 years is 65% and 51% respectively. Metastasis at diagnosis (5 years survival 22% vs 63%, p=0.02) and poor response to initial therapy (5 years survival 54% vs 2 years survival 20%, p=0.01) were found to be poor prog-nostic factors. Age, gender, localisation, osseous/extraosseous tumour and tumour size were not found to be prognostic factors. in conclusion, in this series, survival of children with ESFT was found to be comparable to other studies. Metastasis at diagnosis and response to initial treatment were the most important prognostic factors.
