Hodgkin hastalığında deneyimlerimiz

Özet

Hodgkin hastalığı yeni tanı alan malign hastalıkların %1-2'sini, tüm lenfoproliferatif hastalıkların %40'ını oluşturur. Çalışmamızda Ankara Onkoloji Eğitim ve Araştırma Hastanesi Hematoloji Kliniği'nde 1993-2001 yıllanrı arasında tedavi ve izlemi yapılmış 145 Hodgkin lenfomalı hasta retrospektif olarak değerlendirildi. Hastaların 78'i (%53.8) erkek, 67'si (%46.2) kadındı. Hastaların yaş aralığı 16-77 (ortalama: 47) yıl bulundu. Ann Arrbor evreleme sistemi dağılımına göre olguların %11.7'si (n: 17) evre I, %34.5 (n:50) evre II, %40.7 (n: 59) evre III, %13.1 (n: 19) evre IV konumunda olduğu saptandı. Yüzyirmidört hastada (%85.5) B semptomları saptandı. Rye sınıflamasına göre histopatolojik alt gruplama da %47.6 (n: 69) miks sellüler tip, %27.6 (n: 40) noduler sklerozan tip, %18 (n: 26) lenfositten zengin tip, %6.8 (n: 10) lenfositten yoksun tip şeklinde saptandı. Tedavilerinde ilk seçenek olarak 117 hastada (%80.7) ABVD kombinasyonu, 28 hastada (% 19.3) ise MOPP protokolü uygulandı. Hastaların 113'inde (%78.7) tam remisyon, 11 hastada (%7.6) parsiyel remisyon elde edildi, 21 hastada (%13.1) ise ilk seçenek kemoterapiye cevap alınamadı. Ortalama izlem süresi 84 ay olan olguların %27.8'de progresyon ve nüks saptandı. Ortanca nüks süresi 14 aydı (3-23 ay). Hastaların 5 yıllık yaşam süreleri %71.4 olarak bulundu, 5 yıllık takipte yaşam süreleri evre I - II'de %85, evre III - IV'te ise %15 olarak saptandı (p=0.03).

Abstract

Hodgkin's disease (HD) accounts for about 1% at newly diagnosed malignant disease and 40% of lymphoproliferative disease. In this study, 145 newly diagnosed Hodgkin's disease cases followed between 1993 to 2001 year in Ankara Oncology Hospital of Hematology Clinic were evaluated. There were 78 men (53.8%) and 67 women (46.2%) between 16 and 77 years old (mean:47). In accordance with the staging system of Ann-Arrbor patients were defined as follows: Stage I, 17 patients (11.7%); Stage II, 50 patients (34.5%); Stage III, 59 patients (40.7%); Stage IV, 19 patients (13.1%). Of the 145 patients 124 had B Symptoms. According to Rye Classification; frequency of histologic subtypes showed 47.6% (69 patients) mixed cellularity, 27.6% (40 patients) nodular sclerosis 18% (20 patients) lymphocyte predominant, 6.8% (10 patients) lymphocyte depletion type. Combination chemotherapy of ABVD was used as first line treatment in 117 patients (80.7%). MOPP chemotherapy was used as first line treatment at 28 patients (19.3%). Complete remission was achieved in 78.7% of patients (113 patients) and partial remission in 7.6% (11 patients) of patients. Response could not be obtained in 13.1% (21 patients) of patients. Disease progression or recurrence was observed in 27.8% of patients in 84 months. Median recurrence was 13.6 months (3-23). Five year survival rate was found 71.4% of all patients, 86% in stage I - II, and 15% in stage III - IV (p=0.03).
