Özet

Penis kanseri Türkiye'de ender görülmesine karşın, dünya genelinde, bazı ülkelerde %10-20'ye ulaşan insidansı nedeniyle önemli bir sağlık sorunu oluşturmaktadır. Genellikle 6. dekadda görülür. Sünnetsizlik, yaşlılık ve kötü hijyen bu tümörlerin gelişimindeki başlıca risk faktörleridir. Smegmanın içerdiği özgül kanserojen maddeler bilinmemekle birlikte, sirkumsizyonun bu karsinojenik etkiyi eradike ettiği görüşü hakimdir. Histolojik olarak tüm penis tümörlerinin %95'ten fazlası skuamöz hücreli kanserdir. İkinci sıklıkta metastatik penil karsinomlar görülür ve çoğunlukla mesane, prostat, böbrek ve rektosigmoid tümör kökenlidir. Onkoloji merkezinde çalışmayan çoğu ürolog, nadiren penis kanserli hasta tedavi eder. Ayrıca penis tümörü, diğer genitoüriner malignansilerden daha seyrek görülür ve tedavisi de her yönü ile güçtür. Kliniğimizde 1985- 1999 yılları arasında dokuzu primer ve dokuzu da metastatik olmak üzere total 18 penis kanserli hasta tedavi edilmiştir. Bu makalede, yedisi skuamöz hücreli kanser ve ikisi de peniste sınırlı Kaposi sarkomu tanılı, 9 primer penil karsinomlu hastadaki deneyimlerimiz sunulmuştur.

Abstract

Cancer of the penis is a rare disease in Turkey. However, it represents a significant health problem with an incidence of 10-20% in some countries. It usually presents in the sixth decade of life. The abscence of circumcision, aging and poor hygiene are the main risk factors. Although the specific carcinogenes in smegma remains unidentified, circumcision eradicates its carcinogenic potential. Over 95% of penile carcinomas are histologically squamous cell carcinoma. Metastatic tumors of the penis often originates as cancers of the bladder, prostate, kidney and rectosigmoid primaries. Most urologists, unless practicing at a major oncology center will infrequently treat a patient with penile carcinoma. Additionally, penis tumor is less common than other genitourinary malignancies and represents formidable challenges. From 1985 to 1999, 18 patients (9 primary and 9 metastatic cases), with a diagnosis of penile carcinoma, were treated at our clinic. In this study, our experience in 9 patients with a diagnosis of primary penile carcinoma; 7 cases of squamous cell carcinoma and two patients with Kaposi sarcoma limited to glans penis is presented.
