Özet
Nötropenik enterokolit sıklıkla kemoterapi alan maligniteli çocuklarda görülen akut, yaşamı tehdit eden bir durumdur. Kemoterapi almayan olgularda görülmesi çok nadirdir ve literatürde yalnızca bir akut myeloblastik lösemi(AML) olgusunda bildirilmiştir. AML-M4 tanısı konulan 11 yaşındaki erkek hasta nötropenik ateş nedeniyle antibiyotik alırken, şiddetli karın ağrısı, ishal ve kusma gelişti ve abdominal ultrasonografi ve tomografi ile nötropenik enterokolit tanısı konuldu. Kan kültüründe Enterococcus faecium üredi. Oral beslenmenin kesilmesi, gastrointestinal dekompresyon, antibiotik tedavisi, parsiyel parenteral beslenme ve kan ürünleri desteği ile tam düzelme sağlandı. Nötropenik enterokolit tedavi almayan lösemi olgularında da ayırıcı tanıda düşünülmelidir.
Abstract
Neutropenic enterocolitis is an acute, life threatening condition, often seen in children with malignancies who are receiving chemotherapy. The occurrence of this complication in the absence of chemotherapy is very rare, with only one reported case in acute myeloblastic leukemia(AML). An 11-year-old boy diagnosed AML-M4 receiving antibiotics due to neutropenic fever developed severe abdominal pain, diarrhea, and vomiting and neutropenic enterocolitis was diagnosed by ultrasonography and tomography. Blood culture grew Enterococcus faecium. This patient was successfully treated with cessation of oral nutrition, gastrointestinal decompression, antibiotic therapy, partial parenteral nutrition and transfusion of blood product. Neutropenic enterocolitis must also be considered in the differential diagnosis of abdominal pain in patients with untreated leukemia.
