Özet
Amaç: Aortanın ana yan dallarını ilgilendiren arteriyovenöz fistüller yüksek debili refrakter kalp yetmezliğine yol açabildikleri gibi, cerrahi onarımları sırasında görülebilecek ciddi kanamalarla ölümcül de seyredebilirler. Materyal ve Metod: Bu çalışmada 1985 - 2000 yılları arasında ameliyat edilen toplam 15 arteriyovenöz fistül olgusu incelenmiştir. Hastaların 10'u (%66.7) erkek ve 5'i (%33.3) kadın olup hasta yaşları 8 ile 56 yıl arasında değişmekteydi. Etiyolojik nedenler posttravmatik (%60), konjenital (%26.7) ve iatrojenik (%13.3) sebepler idi. Fistüllerin dokuzu iliak veya femoral arterden, dört tanesi subklavyan arter ve dallarından, bir tanesi karotid arterden ve sonuncusu da vertebral arterden kaynaklanmaktaydı. Kesin tanı arteriyografi veya MR-anjio ile kondu. Beş olguda (%33.3) psödoanevrizma ek lezyon idi. Cerrahi onarım olarak altı hastada (%40) primer onarım, altı hastada (%40) greft interpozisyonu ve üç hastada (%20) embolizasyon uygulandı. Bulgular: Erken veya geç dönem mortaliteye rastlanmadı. Rekürrans iki olguda (%13.3) görüldü ve bu hastalar tekrar ameliyat edildi. Sonuç: Basit olgularda embolizasyon tercih edilebilen bir teknik olmakla birlikte, komplike vakalarda fistül mutlaka çıkarılmalı veya greft interpozisyonu ile arteriyel kaynak kapatılmalıdır.
Abstract
Background: Arteriovenous fistulae of the major branches of the aorta can cause refractory congestive heart failure and it's surgical repair can be mortal due to severe bleeding. Methods: Fifteen patients aged between 8 and 56 years were operated for arteriovenous fistulae between 1985 and 2000. Ten of them were male and the rest female. Trauma was the major cause (60%) of arteriovenous fistulae, where congenital (26.7%) and iatrogenic causes (13.3%) were the other etiologic factors. Nine of them originated from iliac or femoral arteries, four of them from subclavian artery or it's branches, one from carotid artery and the last from vertebral artery. All of them were diagnosed using arteriography or MRI-angio. An associated false aneurysm was detected in five patients. Primer closure was applied in six patients (40%), graft interposition in six patients (40%) and embolization in three patients (20%). Results: There was no early or late mortality. We observed recurrence in two patients (13.3%), who were reoperated. Conclusions: Embolization can be performed in the simple cases, but resection of the fistulae should be performed with/without graft interposition in complicated cases.
