Özet

Akciğer kanseri ve mediastinal tümörlerde superior vena kavanın (SVC) invazyonu; prognozun kötülüğü, rekonstrüksiyon için ideal greft materyalinin olmaması, SVC’nın klemplenmesinin fizyolojik sakıncaları, greft trombozu ve enfeksiyon riski nedeniyle cerrahi için kesin kontrendikasyon olarak belirtilmiştir. Bu yazıda tüm SVC’yi invaze eden akciğer kanserinde torakotami yolu ile akciğer rezeksiyonu ve intraperikardiyal endovenöz şant ile parsiyel SVC rezeksiyonu cerrahi girişimi tarif edilmektedir.

Abstract

In lung cancer and mediastinal tumour surgery, the invasion of superior vena cava (SVC) is reported as the absolute contraindication due to its poor prognosis, the absence of ideal prosthetic material, the phsiological consideration of clamping of SVC, thrombosis of the graft and the risk of infection. We reported a case in which resection of SVC was performed by endovenous shunt procedure
