Greft trombozuna hibrid cerrahi yaklaşımı

Özet

Bu yazıda, aortobifemoral baypas greft uygulananbacağında tromboz görülen bir olguda, farmakomekaniktrombektominin ardından yapılan periferik vaskülercerrahi deneyimimiz sunuldu. Kırk dokuz yaşındakierkek hastanın sol ayak serçe (5.) parmağında nekrotikyarası vardı. Anjiyografide aortobifemoral baypas greftinsol bacağının tıkalı olduğu görüldü. Spinal anestezialtında klasik embolektomi işleminin başarısız olmasıüzerine, Cleaner trombektomi kateteri kullanılaraktrombektomi uygulandı. Greft profunda femoral artereanastomoze edildi, ipsilateral bacaktan çıkarılan safenven ise, greft ile infragenikular popliteal arter arasınainterpoze edildi. Klasik embolektominin yetersizkaldığı olgularda farmakomekanik trombektomi ilebirlikte uygulanacak hibrid cerrahi girişimlerin greftve ekstremitenin kurtarılmasında bir tedavi seçeneğiolacağını düşünüyoruz.

Abstract

In this article, we present our experiences on peripheralvascular surgery following pharmacomechanicalthrombectomy in a case of thrombosis in his leg, which wasperformed aortobifemoral bypass graft. Forty-nine-year-oldmale patient had a necrotic wound at his left foot little(5th) finger. Angiography revealed a blockage in the leftleg of aortobifemoral bypass graft. When conventionalembolectomy procedure under the spinal anesthesiafailed, thrombectomy was performed with a Cleanerthrombectomy catheter. Graft was anastomosed to arteriaprofunda femoris and the saphenous vein harvested fromthe ipsilateral lower extremity was interpositioned betweenthe graft and infragenicular popliteal artery. We believethat the hybrid surgical interventions in combination withpharmacomechanical thrombectomy are useful treatmentof choices to save the graft and extremity in cases in whomconventional thrombectomy is inadequate.
