Özet

Bu çalışma medikal tedavi ile tedavi edilen akut koroner sendrom (AKS) hastalarında klopidogrelin hastane mortalitesine etkilerini araştırmak amacıyla planlandı. 3024 olgunun kaydında, hastalar kararsız angina pectoris (UAP), ST yükselmesiz miyokard infarktüsü (NSTEMI) ve ST yükselmeli miyokard infarktüsü (STEMI) olarak gruplara ayrıldı. İlk iki grupta, hastaların heparin, asetil şahsilik asit, beta bloker ve nitratlardan oluşan standart tedavi ve klopidogrel yada sadece standart tedavi alıp almamasına göre iki alt grup oluşturuldu. STEMI hastalarında, hastaların trombolitik, standart tedavi ve klopidogrel alıp almamalarına göre dört alt grup oluşturuldu. Tüm grupların hastane mortaliteleri karşılaştırıldı. UAP grubunda, 711 hastada, mortalite klopidogrel alanlarda %1.4 ve almayanlarda %2.3 bulundu (p>0.05). NSTEMI grubunda, 1326 hastada, mortalite klopidogrel alanlarda %2.5 ve almayanlarda %4.2 bulundu (p0.05). Abstract

To evaluate the effect of clopidogrel on in-hospital mortality in patients with acute coronary syndrome (ACS) treated with medically. In the registry of 3024 cases, patients were divided as unstable angina pectoris (UAP), non-ST segment elevation myocardial infarction (NSTEMI) and ST-segment elevation myocardial infarction (STEMI). Two subgroups of first two groups were constituted whether patients treated with standard therapy (heparin, acetyl salicylic acid, beta-blocker, nitrates) and clopidogrel or patients treated with only standard therapy. In STEMI patients four subgroups were constituted whether patients were treated with thrombolytic, standard therapy and clopidogrel. In-hospital mortality rates of all groups were compared.In UAP, of 711 patients, mortality were found 1.4 % in clopidogrel group and 2.3 % not taking (p>0.05).
