Bu çalışmamızda sol ventrikül diyastolik fonksiyon bozukluğu (SVDDF) tanısı almış hastalarda sistolik ve diyastolik fonksiyonları birlikte test eden yeni bir parametrenin klinik önemini araştırmak istedik.Hastalar sol ventrikül (SV) doluş akımlarına ve nabız dalgalı doku Doppler (pw-TDI) bulgularına göre 4 gruba ayrıldılar. Grup 1 (n=112) normal diyastolik fonksiyonları temsil ederken, grup 2 (n=128) bozulmuş SV relaksazyonunu, grup 3 (n=46) pseudonormalizasyonu ve grup 4 (n=15) ise reversible restriksiyonu gösteriyordu. Test ettiğimiz yeni indeks (indeks =Ea-Aa/Sa) ise pw-TDI parametrelerinden erken diyastolik velositesinin geç diyastolik velositeden çıkartılıp sistolik velositeye bölünmesinden oluşmaktaydı. Sistolik ve diyastolik fonksiyonları birlikte test eden parametreler arasında yeni indeks ve Tei indeksinin kontrol grubunda diğer gruplara göre farklı olduğu görüldü. Abstract
We aim to investigate the clinical significance of a new index that evaluates systolic and diastolic functions together in left ventricular diastolic dysfunction (LVDDF). Cases were divided into four groups according to their left ventricular (LV) inflow and pulsed wave tissue Doppler imaging (pw-TDI). Group 1 (n=112) represented normal diastolic LV inflow while group 2 (n=128) represented impaired LV relaxation, group 3 (n=46) represented pseudonormalization and group 4 (n=15) represented reversible restrictive LVDDF. We defined a new index (novel index=Ea-Aa/Sa ) by using the pw-TDI parameters taken from LV basal-lateral annulus and subtracting late (Aa) velocity from early (Ea) velocity and dividing it by systolic (Sa) velocity. Among the indices that evaluate systolic and diastolic functions together, novel index and Tei index were lower in group İ (control group) compared to other groups.
