Benign prostat hiperlazi'li hastalarda cinsel fonksiyonlar prostatektomi'den etkilenir mi?

Özet

Benign prostat hiperplazisi yaşlı hastalarda görülen yaygın bir ürolojikproblemdir. Alt üriner sistem semptomları sıklıkla bu hastalık ile ilişkilidirve hayat kalitesi üzerinde olumsuz etkiye sahiptir. Günümüzde, benign prostathiperplazisi hastaları için medikal ve cerrahi olmak üzere iki temel tedaviyöntemi vardır. Cerrahi tedavi pek çok hastada yapılmaktadır. Hastalarınçoğunluğu cerrahi yöntemlerden oldukça yarar görmesine rağmen, bu yöntemlerincinsel fonksiyonlar üzerine olumsuz etkileri bulunmaktadır. Biz, benign prostathiperplazili hastalarda cerrahi tedavinin cinsel fonksiyonlar üzerindekietkisini değerlendirdik. Alt üriner sistem semptomları nedeniyle kliniğimize başvuran 50 hasta çalışmaya dahil edildi. Tüm hastalar normal parmak ile rektalmuayene ve serum prostat spesifik antijen seviyelerine sahipti. Transüretralprostat rezeksiyonu ve açık prostatektomi sırasıyla 34 (%68) ve 16 (%32) hastadayapıldı. Hastalarımızda libido, erektil fonksiyon, ejakulasyon ve cinselilişki sıklıkları sırasıyla %32, %42, %46 ve %52 oranlarında cerrahiden olumsuzetkilenmesine rağmen ne trans üretral prostat rezeksiyonu ne de açıkprostatektominin cinsel fonksiyon parametreleri üzerinde bir seçiciliğisaptanmamıştır (p=0.058). Alt üriner sistem semptomları olan hastalarprostatektomi’nin cinsel fonksiyonlar üzerine olan etkileri hakkındabilgilendirilmelidir.

Abstract

Benign prostate hyperplasia is a common urological problem seen in elderlypatients. Lower urinary tract symptoms are commonly related to the disease andhave negative effect on the quality of life. Nowadays, there are two maintreatment modalities asmedical and surgical for benign prostate hyperplasiapatients. Surgical treatment are performed in many patients.Although most of the patients greatly benefited from the surgical methods,they also have negative effects on sexual functions. We assessed the efficacyof surgical treatment on sexual functions in patients with benign prostatehyperplasia. Fifty patients who were admitted to our clinic for lower urinarytract symptomsms were involved in the study. All patients had normal digitalrectal examination and serum prostate specific antigen levels. Transurethralresection of the prostate and open prostatectomy were performed in 34 (68%)and 16 (32%) patients, respectively. Although libido, erectile function,ejaculation and sexual intercourse rates were negatively affected in ourpatients by the surgery in the ratio of 32%, 42%, 46% and 52%, respectively,neither trans uretrhral resectionaf prostate nor open prostatectomy showed anyselectivity on the sexual function parameters (p=0.058). The patients withlower urinary tract symptoms should be informed about effects of prostatectomyon sexual functions.
