Huzurevinde yaşayan yaşlı popülasyonda kas-iskelet sistemi sorunlarının değerlendirilmesi

Özet

Giriş: Kas-iskelet sistemi hastalıkları yaşlı popülasyonun kendine yeterliliğini ve yaşam kalitesini bozabilmektedir. Bu tür hastalıkların erken tanısı ve tedavisi mümkündür. Bu çalışmada, ilimiz huzurevinde yaşayan yaşlılarda mevcut kas-iskelet sistemi hastalıkları araştırılarak, bu hastalıkların neden olduğu morbidite ve mortalitenin önlenmesine yönelik tedavi ve rehabilitasyon yöntemlerinin vurgulanması amaçlandı.Gereç ve Yöntem: Huzurevinde yaşayan 90 yaşlı gönüllü (29 kadın, 61 erkek) çalışmaya dahil edildi. Katılımcıların ad-soyad, yaş, meslek, huzurevinde kalma süresi, alışkanlıkları, hastalıkları ve kullandıkları ilaçlar sorgulanarak kaydedildi, kas-iskelet sistemi muayeneleri yapıldı.Bulgular: Çalışma grubumuzun yaş ortalaması 77±8 (61-100) idi. Çalışmaya dahil edilen yaşlılarda; %28 lomber spondiloz, %26 gonartroz, %16 servikal spondiloz, %14 koksartroz, %13 lomber radikülopati, %10 kalça protezi, %10 hemipleji, %7 servikal radikülopati, %4 ekstremite ampütasyonu, %3 rotator kaf lezyonu, %3 diz protezi saptandı. Yaşlıların %9'u yatalak olup, % 60'ında birden fazla kas-iskelet sistemi şikayeti mevcuttu.Sonuç: Huzurevinde kalan yaşlıların %80'ninde fizik tedavi ve rehabilitasyon gerektiren kasiskelet sistemi problemleri saptandı. Huzurevinde yaşayan ve çalışmamıza dahil edilen kişiler düzenli fizik tedavi, rehabilitasyon veya egzersiz programı alamıyorlardı. Kişilerin sağlık problemlerine yönelik tedavi önerilerinde bulunuldu. Bu tür merkezlerde, yaşlılara aktif hizmet ve eğitim veren fizik tedavi üniteleri kurulmasının, yaşlının kendine yeterliliğini ve yaşam kalitesini arttırabileceği düşüncesine varıldı.

Abstract

Introduction: Musculoskeletal disorders in the elderly may impair self-sufficiency and quality of life. Early diagnosis and treatment of such diseases are possible. The aim of our study was to accentuate the methods of treatment and rehabilitation for preventing morbidity and mortality due to the existing diseases of the musculoskeletal system, detected in the elderly living in the nursing home of our city.Materials and Method: Ninety elderly volunteers (29 women, 61 men) living in the nursing home of our city were included in the study. Name and surname, age, occupation, duration of living at rest home, habits, diseases and use of medications were questioned and recorded for all of the participants and musculoskeletal examination was carried out.Results: Mean age of our study group was 77±8 years (range: 61-100). Frequently observed disorders were as follows: lumbar spondylosis (28%), knee osteoarthritis (26%), cervical spondylosis (16%), coxarthrosis (14%), lumbar radiculopathy (13%), hip prosthesis (10%), hemiplegia (10%), cervical radiculopathy (7%), extremity amputation (4%), rotator cuff lesions (3%), knee replacement (3%), bedridden (9%). In 60% of the elderly more than one musculoskeletal system complaints were found.Conclusion: Musculoskeletal problems requiring physical therapy and rehabilitation were detected in 80% of the elderly living in the nursing home. The elderly people living in the nursing home included in this study could not receive regular physical therapy, rehabilitation or an exercise program. Recommendations were made to the elderly for the treatment of health problems. In our opinion, establishment of physical therapy units in such centers and providing continuous active service and training to the elderly may increase self-sufficiency and quality of life.
