ERKEN EVRE DİZ OSTEOARTRİTLİ HASTALARDA POSTÜRAL STABİLİTENİN SAĞLIKLI BİREYLER İLE KARŞILAŞTIRILMASI

Özet
Amaç: Çalışmanın amacı, erken evre (1-2) diz osteoartriti (OA) olan hastalarda sağlıklı bireyleregöre dengenin etkilenip etkilenmediğinin incelenmesi ve erken evre diz OA olan hastalarda dengeile diz fonksiyonu, ağrı şiddeti ve vücut kütle indeksi arasındaki ilişkinin araştırılmasıydı.Yöntem: Bu çalışma, 20 kadın ve dokuz erkek olmak üzere 29 diz OA’lı hastanın katılımı ilegerçekleşti. Sağlıklı kontrol grubu ise, 24 kadın, altı erkek olmak üzere 30 asemptomatik bireydenoluştu. Çalışmaya katılan bireylerin dengesi Biodex Denge Sistemi (BDS) kullanılarak, ağrı şiddetiVizüel Analog Skalası (VAS) ile, diz fonksiyonları ise, Westhern Ontario Osteoartrit İndeksi (WOMAC)ile değerlendirildi.Sonuçlar: Her iki grubun gözler açık pozisyonda postüral stabilite test sonuçlarının benzer olduğubulundu (p>0,05). Gözler kapalı pozisyonda ise, diz OA olan hastaların anterioposterior (p=0,013),mediolateral (p=0,024) ve toplam (p=0,001) postüral stabilite test puanlarının kontrol grubuna görearttığı bulundu. Diz OA olan hastalarda gözler kapalı pozisyonda mediolateral postüral stabilite testpuanı ile WOMAC arasında ilişki olduğu bulundu (r=0,687-0,615, p0,05).Tartışma: Erken evre diz OA olan bireylerde gözler kapalı pozisyonda postüral salınım miktarısağlıklı bireylere göre artar ve bu artış fonksiyonel düzeydeki kayıpla ilişkilidir. Diz OA’da erkendönemden itibaren denge etkilenmeye başlar ve bu yüzden dengeyi geliştirmeye yönelik girişimlertedavi programına dahil edilmelidir.
Abstract
Purpose: The study aimed to analyze whether the balance is affected in early stage (1-2) knee osteoarthritis (OA) as compared to healthy individuals and to investigate the relationship between balance and knee function, pain intensity, and body mass index in early-stage knee OA. Methods: This study was carried out with the participation of 29 knee OA patients, including 20 females and nine males. The healthy control group consisted of 30 asymptomatic individuals, including 24 females and six males. The balance of the participants was evaluated using the Biodex Balance System (BDS), the pain intensity was assessed using Visual Analogue Scale (VAS), and the knee function was determined using the Western Ontario Osteoarthritis Index (WOMAC). Results: Postural stability test results in eyes open position were similar between the groups (p>0.05). In the eyes closed position, anteroposterior (p=0.013), mediolateral (p=0.024) and total (p=0.001) scores of OA patients increased as compared to the control group. Postural stability test scores of patients with knee OA were found increased compared to the control group. A correlation was found between WOMAC and mediolateral postural stability test score in eyes closed position (r=0.687-0.615, p0.05). Conclusion: The amount of postural oscillation in early knee OA patients in eyes closed position is increased compared to healthy individuals, and this increase is associated with the loss in functional level. Balance begins to be affected from the early stage of knee OA, and therefore attempts oriented to improve the balance should be included in the treatment program.
