Diz altı amputelerde farklı postoperatif ödem kontrol yöntemlerinin etkinliğinin karşılaştırılması

Özet
Amaç: Amputelerde ödem kontrolünde elastik bandaj (EB) yöntemi yaygın olarakkullanılmaktadır. Son yıllarda, ödem tedavisinde etkin bir yaklaşım olarak kompleks boşaltıcıfizyoterapi (KBF) uygulamaları da yapılmaktadır. Çalışmamız, diz altı amputelerde EB ve KBFyöntemlerinin, ödem kontrolü ve kalıcı proteze geçiş süresine etkilerini karşılaştırmak amacıylaplanlandı. Yöntem: Çalışmaya, 20 unilateral travmatik diz altı ampute dahil edildi. Amputelergeliş sırasına göre randomize edilerek, 10 ampute EB grubuna, 10 ampute KBF grubuna alındı.Uygulamalar öncesi ve sonrasında güdük ve sağlam taraftan çevre ölçümleri yapıldı. Hastanedekalış, kalıcı proteze geçiş, ilk soket değişim süresi, güdük ağrısı ve fantom ağrısı kaydedildi.Sonuçlar: EB ve KBF grubunda hastanede kalış süreleri farklı bulunmazken (p>0.05), KBFgrubunda kalıcı proteze geçiş süresi daha kısa ve kalıcı protezde ilk soket değişim süresinindaha uzun olduğu belirlendi (p0.05), diğerölçümlerde gruplarda fark bulundu (p0.05). Tartışma: KBF uygulaması ile kalıcı proteze geçişsüresinin kısalması ve ilk soket değişim süresinin uzaması, bu yöntemin diz altı amputelerdepostoperatif dönemde etkili bir tedavi yöntemi olarak kullanılabileceğini göstermektedir.
Abstract
Purpose: The classical elastic bandage (EB) application is a common method used in edemacontrol in amputees. In recent years, however, complex decongestive physiotherapy (CDP) isfound to be an effective approach in the treatment of edema. This study was planned tocompare effectiveness of EB and CDP technique on the edema control and the period oftransition to permanent prosthesis in below knee amputees. Methods: Twenty unilateral belowknee amputees were included in the study. Ten amputees were allocated to the EB group andthe other 10 were allocated to the CDP application with randomization. Intact and amputatedlimbs were assessed using pre and post treatment circumferential measurements in bothgroups. The duration of stay in the hospital, duration of transition to permanent prostheses,duration of the change in the first socket, phantom limb pain and stump pain were alsorecorded. Results: Significant differences were found in terms of transition period topermanent prosthesis and duration of the change in the first socket in favor of CDP group(p0.05). Circumferential measurement comparisons did not show any difference between thepre and post treatment of the mid-thigh results in CDP group (p>0.05), while all othermeasurements showed differences (p0.05). Conclusion: Reducing the transitionperiod to permanent prosthesis and duration of the change in the first socket with CDP showedthat CDP may be used as an effective method for below knee amputees.
