Özet

AMAÇ: Postmenopozal kadınlarda hormon replasman tedavisi (HRT) süresince meme dokusunda yeni benign ve/veya malign oluşumları belirlemek ve HRT'nin meme kanseri üzerine etkisini literatür eşliğinde değerlendirmek. MATERYAL METOD: Ocak 2001- Mart 2002 tarihleri arasında HRT uygulanan 1900 kadın çalışmaya dahil edildi. Olgular tedavi başlangıcında mamografi ve/veya meme ııltrasonografÎ ile değerlendirildi. Bir yıl sonra tetkikler tekrarlandı. Meme dokusunda yeni gelişen solid ve/veva kistik oluşumlar saptandı. Oluşumlar ince iğne aspirasyou biopsisi ve/veva total eksizvonel biopsi ile incelendi. Bu olgular yaş, menopoz süresi. HRT tipi, HRT süresi, vücut kitle indeksine göre değerlendirildi. BULGULAR: Ondört aylık izlem süresince HRT alan 1900 kadın incelendi. Bu olguların kullanılan ajana Olgular HRT tipine göre üç gruba ayrıldı: 800 hasta Ostrojen + Progesteron (E+P) grubunda, 600 olgu Ostrojen (E) grubunda, 500 olgu Tibolon (T) grubunda, yer aldı. HRT sırasında 1900 olgu içerisinde 20 olgu solid, 15 olgu kistik olmak üzere toplam 35 olguda kitle saptandı (% 1,3 ). Kitlelerin gruplara göre dağılımı E+P: 10, E: 13 T: 12 idi. Patolojik değerlendirme sonucunda iki olguda grade l duktal karsinoma saptandı, ilk olgu tibolon grubunda, 59 yaşında, nullgravid, 9 yıldır spontan menopozda, 1 yıldır ilaç kullanmaktaydı. Tedaviye başlanması sırasında çekilen mamografide heterojen fibroliposkleroze meme paterni, meme USG'de bilateral liposkleroze doku saptanmıştı, ikinci olgu 63 yaşında, gravida 3, II yıldır cerrahi menopozda, 6 aydır Ostrojen (Estradiol) kullanmaktaydı. Tedavi başlangıcında çekilen mamografide liposkleroze meme patenti, meme USG'de fibrokistik patern saptanmıştı. Hastanın ağrı şikayeti nedeniyle yapılan değerlendirme sonrasında 6 ay sonra tetkikler tekrarlandı. SONUÇ: HRT süresi meme kanseri gelişiminde önemli risk faktörlerinden biridir. Progestajenik ajanlar endoınetrium üzerinde protektif etki göstermelerine rağmen, bu etki meme üzerinde görülmemektedir. Bundan dolayı bu ajanların HRT'de kullanımında dikkatli olunmalıdır.

Abstract

OBJECTIVE: To determine the new benign and/or malign formations in the breast tissue in postmenopausal women with hormone replacement therapy ( HRT) and to evaluate the effect of HRT on breast cancer according the literature. MATERIALS AND METHODS: 1900 women with HRT were included in this study between January 2001 to March 2002. In the beginning, women were evaluated with mammography and/or breast ultrasonography. The new solid and/ or cystic formations in the breast tissue were determined by annual mammography and/or breast sonography. Fine needle aspiration biopsy or total excisional biopsy was performed whenever Indicated.The patients with a new lesions were evaluated for age, menopausal period, the type of HRT. duration of HRT and body mass index. RESULTS: During the fourteen month follow up period, 1900 women with HRT were evaluated. In releate to the type of the HRT, the patients were divided into three groups: 800 patients were an Betrogen + Progestagen (E+P), 600 patients were an Estrogen (E), 500 patients were an Tibolone (T) replacement therapy. During these three type of HRT. 20 solid and 15 cystic developments in the breast tissue were detected in 35/1900 women ( %l.8). 10/35, 13/35 and 12/35 were in E+P, E and T group, respectively. Two ductal carcinoma were reported by their histopathological analysis. One patient with breast cancer who was on tibolone treatment for one year, 59 years old, nulligravid and her spontaneous menopause duration was 9 years. Her baseline mammogram shown heterogen fibroliposclerosis and the breast sonography revealed bilateral liposclerosis in the breast tissue. The second patient was 63 years old, gravida 3, and had surgical menopause for II years. She was on the oestragen (Estradiol) therapy for six month. The baseline mammography shown bilateral liposclerosis and breast sonography revealed fibrocystic pattern. Because of the pain complaint, this patient was reevaluated 6 months later. CONCLUSION: HRT duration is one of the important factors on the formation of breast cancer. The use of progestagenic agents showed a protective effect on the endometrium but not on the breast tissue. So the use of these agents with HRT requires prime care.
