Orta-serebral doppler ultrasonografi ile fetakl parvovirüs nonimmun hidrops fetalis olgusunun takibi: Bir olgu sunumu

Özet
Giriş: İntrauterin fetal parvovirus enfeksiyonuna bağlı non-immun hidrops fetalis tanısı ile kliniğimize başvuran bir gebede orta serebral arter pik akım hm (MCA PSV) ile fetal anemi takibi ve tedavisi sunulmuştur. Olgu Sunumu: 22 haftalık gebelik ve non-immune hidrops fetalis tanısı ile kliniğimize sevkedilen hastanın yapılan MCA PSV incelemesinde akım hızının artmış olduğu saptandı ve fetal anemi düşünüldü. Yapılan fetal ultrasonografi ve ekokardiyografide ek fetal anomali saptanmadı. Annede yapılan parvo virüs IgG ve IgM düzeylerinin pozitif gelmesi üzerine kordosentez yapılarak hidrops sebebi olabilecek fetal bulguların değerlendirilmesine ve transfûzyon yapılmasına karar verildi. 24. haftada birinci transfûzyon fetal hidrops nedeniyle 15 cc ile sınırlı tutuldu. Bir hafta sonra, ikinci transfûzyon yapıldı, ikinci transfüzyonda 45cc O Rh(-) kan verildi. Haftalık doppler ile takip edildi. MCA PSV sonuçları normal olduğu sürece kordosentez ve İntrauterin fetal transfûzyon yapılmadı. 37 haftada hemoglobin düzeyi 17gr/dl olan sağlıklı bir kız bebek doğurtuldu. Tartışına ve Sonuç: Parvo virüs enfeksiyonuna bağlı non-immune hidrops ile başvuran olgularda fetal aneminin değerlendirilmesinde ve takibinde orta serebral doppler akımının yol gösterici olabilir.
Abstract
Introduction: Diagnosis of anemia and follow-up by middle cerebral peak systolic velocitv (MCA PSV) is presented in a patient with non-imune hydrops due to in-utero fetal parvo virus injection. Case: A patient at 22 weeks with non-immune hydrops was presented with increase in MCA PSV suggesting severe fetal anemia. Maternal Parvo virus IgM and IgG titers were elevated. In order to evaluate the likely etiology and treat fetal anemia, cordocentesis and transfusion were performed. Unless MCA PSV was increased for the gestational age intrauterine invasive testing was avoided. First transfusion was limited to I See due to hydrops. After a second transfusion of 45 cc at 24 weeks, patient did not require any other transfusions as indicated by doppler ultrasound. A healty girl with hb of 17 gr/dl was delivered at 37 weeks by cesarean section. Conclusion: MCA PSV mav be used for the evaluation and follow-up of fetal anemia in patients with non-immune hydrops fetalis due to in utero Parvo virus infection.
