Geçmişte ve halen hormon replasman tedavisi kullanan postmenopozal kadınlarda mammografik dansite artışı

Özet
Amaç: Hormon replasman tedavisi (HRT) kullanan, geçmişte HRT kullanıp bırakmış ve HRT tedavisi almamış hastalarda, mammografik meme paternlerini inceleyip karşılaştırmak. Materyal ve Metod: Ankara Numune Hastanesi Radyoloji Kliniğine mammografi için başvuran 715 postmenopozal kadın çalışmaya dahil edildi. Hormon replasman tedavisi hiç kullanmamış, geçmişte kullanmış ve halen kullanmakta olan hastaların mammografileri Wolfe klasifikasyonuna göre sınıflandırılıp karşılaştırıldı. Bulgular: Geçmişte HRT kullanmış hasta grubunda, halen HRT kullanmakta olan gruba göre N1 paternine daha çok, DY paternine daha az rastlansa da, gruplar arasında anlamlı bir fark saptanmadı. Ancak bu iki gruptaki N1 ve DY paterni dağılımları, hiç HRT kullanmamış hasta grubundan anlamlı olarak farklıydı (p0.05). Sonuçlar:HRT mammografik dansitede artışa neden olmaktadır ve bu ilişki dinamiktir. HRT’nin kesilmesiyle mammografik dansite anlamlı olmasa da azalmaktadır. Ancak daha önceki çalışmalarda belirlenen, tibolon ve siklik-ardışık tedavinin daha az oranda meme dansite artışına neden olduğu iddiası ise çalışmamızda desteklenememiştir. Bu konunun açığa kavuşması için daha geniş bir populasyonda, prospektif bir çalışma yapılması gerekmektedir.
Abstract
Objective: Comparison of mamographic patterns between the current hormon replacement therapy (HRT) users, non-users and past users. Materials and Methods: Sevenhundred and fifteen consecutive women admitted for mamography were enrolled in this study. Mamographies of current HRT users, non-users and past users were classified and compared according to Wolfe classification. Results: N1 pattern was detected more frequently in the past HRT user than current user. Although DY pattern was less frequent in the past user group, this relationship was not statistically significant. The distribution of N1 and DY patterns in the current user and non-user group were different from the past user (p0,05). Conclusion:HRT causes increased mamographic density and this relationship is dynamic process. Following the cessation of HRT mamographic density decrease however it is not statistically significant. Our study did not support an opinion in the previous reports such as tibolon and sequential cyclic regimens do not cause increased mamographic density. We need further prospective studies in the larger population for clear definition of this issue.
