Postmenopozal hastalarda hormon replasman tedavisi ve tibolonun kemik mineral yoğunluğu ve serum lipidleri üzerine etkilerinin karşılaştırılması

Özet

Amaç: Postmenopozal dönemde tibolon, konjuge östrojen (KÖ) + medroksi-progesteron asetat (MPA) ve transdermal olarak sadece östrojen tedavisi uygulanan kadınlarda kemik mineral yoğunluğu, serum kalsiyum, alkalen fosfataz ve serum lipid düzeyleri üzerine olan etkilerini incelemek ve karşılaştırmak. Materyal ve Metod: Bu çalışma Cumhuriyet Üniversitesi Hastanesi Menopoz polikliniğine Ocak 2001 ile Temmuz 2002 tarihleri arasında daha önce hormon replasman tedavisi almamış 66 postmenopozal kadın üzerinde yapıldı. Çalışmaya alınan kadınlar tibolon (n=28), KÖ+MPA (n=19) ve sadece östrojen (n=19) grubu olarak üçe ayrılarak incelendi. Tüm kadınların yaşları, gebelik sayıları, menopoz yaşları, menopoz süreleri, vücut kitle indeksleri ve sigara kullanım oranları kaydedildi. Tedavi öncesi ve sonrası tüm kadınların lumbar vertebra ve feroral kemik mineral yoğunlukları, serum kalsiyum ve ALP (Alkalen fosfataz) düzeyleri ve LDL-Kolesterol (LDL-K), HDL-Kolesterol (HDL-K), Total Kolesterol ve trigliserid düzeyleri saptandı. İzlem süresi ortanca 12 (6-18) aydı. Verilerin karşılaştırılması tek (çalışma grubu) ve çift faktörlü (çalışma grubu ve ölçüm zamanı) ANOVA, bağımlı grup t testi ve ki-kare testi ile yapıldı. Bulgular: Çalışmaya alınan kadınların demografik özellikleri incelendiğinde tibolon grubundaki kadınların yaşlarının ve menopoz yaşları ortalamasının KÖ+MPA grubununkinden anlamlı olarak yüksek olması (50.9±5.5 karşı 46.2±4.9) dışında önemli bir farka rastlanmadı. Tibolon grubunun tedavi sonrası kalsiyum düzeyi KÖ+MPA grubununkinden anlamlı olarak yüksek bulundu (9.1±0.4 mg/dl - 8.8±0.3 mg/dl). Tibolon grubunun tedavi sonrası trigliserid düzeyi transdermal östrojen grubununkinden anlamlı olarak daha düşük bulundu (114.1±51.5 - 162.1±82.0). Tibolon grubunda tedavi sonrası ALP ve LDL-K düzeyi tedavi öncesine göre anlamlı olarak daha düşük ve HDL-K düzeyi tedavi öncesine göre anlamlı olarak daha yüksek bulundu (ALP için 81.4!21.4 U/L 74.3!20.1 U/L, LDL-K için 137.8±44.1 mg/dl - 121.3±41.2 mg/dl, HDL-K için 46.8!15.9 mg/dl - 50.4±10.2 mg/dl. p

Abstract

Objective: To investigate and compare the effects of serum calcium, alkaline phosphatase (ALP) and serum lipid levels, bone mineral density in the postmenopausal women using tibolon, conjugeted estrogen (CE) + medroxy - progesteron asetat (MPA) and only in transdermal estrogen. Materials and Methods: This study is applied to 66 postmenopausal women that have not taken hormon replacement therapy (HRT) between January 2001 and July 2002 at the Cumhuriyet University Hospital of Menopause Department. The applied women taken in to study are divided in to 3 groups; tibolon (n=28), CE+MPA (n=19) and only estrogen group (n=19). All women’s ages, parite numbers, menopause ages, menopause times, body mass index and smoking rates are recorded. After and before the treatment all women’s lumbar vertebral and femoral neck bone mineral density serum Calcium, ALP, LDL - Cholesterol (LDL - C), HDL - Cholesterol (HDL - C), Total Cholesterol and trigliserid (TG) levels are recorded. The following period is mean 12 (6-18) months. The comparison of the results are made by one way ANOVA (investigation group and two way ANOVA (investigation group and measure time), related group t test and c square test. Results: When demografic features of women’s taken this study is investigated; means of ages and menopause ages of women’s in Tibolon group was higher than that of CE + MP A group. (50.9 ct 5.5 to 46.2 ct 4.9). Among this demographic features there were not any important difference. Calcium level of Tibolon group after treatment was found significantly higher than CE + MP A groups (9.1 ± 0.4 mg/dl versus 8.8± 0.3 mg/dl). TG level of Tibolon group after treatment was found significantly lower than transdermal estrogen group (114.1 ± 51.5 versus 162.1 ± 82.0). ALP and LDL - C levels of Tibolon group after treatment was found significantly lower than before treatment and HDL - C level was found significantly higher than before treatment (for ALP 81.4± 21.4 D/L versus 74.3± 20.1 D/L; for LDL-C 137.8± 44.1 mg/dl versus 121.3± 41.2 mg/dl; for HDL- C 46.8± 15.9 mg/dl versus 50.4± 10.2 mg/dl. p
