Medroksiprogesteron asetatin perimenopozal dönemdeki hastalarda psikosomatik semptomlar üzerine olan etkileri

Özet
Amaç: Yaklaşık iki yıl kadar süren perimenopozal dšnemde, psikosomatik semptomların tedavisinde medroksiprogesteron asetat tedavisinin etkinliğini incelemekÇalışmanın yapıldığı yer: Gazi Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum ABDMateryal - Metod: Gazi Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum ABD polikliniğine, 2001 Mayıs-2002 Ekim tarihleri arasında, oligomenoreik mens ve sıcak basmaları nedeniyle başvurmuş perimenopozal dšnemde olan toplam 40 hasta çalışmaya alınmıştır. Hastaların sıcak basmalarına göre sınıflandırılması olmayan, hafif, orta, şiddetli olmak üzere dörde ayrıldı. Altı aylık siklik medroksiprogesteron asetat tedavisi uygulandıktan sonra hastaların semptomları tekrar değerlendirildi.Bulgular: Tedavi öncesi 9 vakada hafif ılıklık hissi, 21 vakada ılıklık hissi ile beraber terleme ve 10 vakada ise terleme ile beraber günlük yaşamında etkilenme tespit edilmiştir. Tedavi sonrasında ise 14 vakada hiç semptom saptanmazken, 26 vakada hafif ılıklık hissi olduğu tespit edilmiştir.Sonuç: Perimenopozal psikosomatik semptomlar üzerinde medroksiprogesteron asetat tedavisi etkili olmakta, bu dönemdeki kadınların sıcak basması şikayetlerini geriletmektedir.
Abstract
Objective: To evaluate the effects of medroxyprogesterone acetate on endometrial estrogen and progesteron receptors in the perimenopausal period of life. Instutition: Gazi University Faculty of Medicine, Obstetrics and Gyneacology Department Material Ð Method: 40 patients who were admitted to our clinic with the complaints of oligomenorrhea and hot flashes between May 2001 and October 2002 were accepted for the study. The patients used 10 mg MPA for six months in a cyclic manner. Before and after the treatment we evaluated the status of endometrial estrogen and progesteron receptors by using pipelle endometrial sampling. Results: The patients were divided in to three groups according to their endometrial histological properties. At the admittance to our clinic; in the first group there were 12 patients whose endometrium were in the phase of the early secretuar endometrium, the second group included 12 patients whose endometrium were in the phase of the late secretuar endometrium, the third group included 16 patient whose endometrium were in the phase of the proliferative endometrium. After six months we reevaluated the status of endometrial estrogen and progesteron receptors. The glandular and stromal estrogen and progesteron receptor scores of endometrium were decreased after the treatment. Conclusion: In the perimenopausal period of life, medroxyprogesterone acetate usage causes a decrease at glandular and stromal estrogen and progesteron receptor scores.
