Özet
Amaç: Bu çalışmada, semen analizi sonrası spermlerin belirgin olarak (>%35) makrosefalik formda olduğu ve bununla birlikte, çok kuyruklu sperm, kuyruksuz sperm ve globozoospermi olarak tanımlanmış sperm içeren vakalarda preimplantasyon genetik tanı (PGT) tekniğinin implantasyon ve devam eden gebelik oranları üzerindeki etkisi değerlendirilmektir.Çalışmanın Yapıldığı Yer: İstanbul Memorial Hastanesi, YÜT, Reprodüktif Endokrinoloji ve Genetik Merkezi, İstanbul Materyal ve Metod: Çalışmada İstanbul Memorial Hastanesi YÜT, Reprodüktif Endokrinoloji ve Genetik Merkezine başvuran ve yardımcı üreme teknikleri (YÜT) programına alınan ve makrosefalik sperm özelliği gösteren 73 çifte ait 83 siklus retrospektif olarak incelenmiştir. 23 siklusta elde edilen embriyolarda, embriyo gelişiminin 3.günü uygun gelişim düzeyinde olanlarına preimplantasyon genetik tanı (PGT) amaçlı biyopsi işlemi uygulanmıştır. Elde edilen blastomerler floresan in situ hibridizasyon (FISH) tekniği kullanılarak 13., 18., 21. X ve Y kromozomları yönünden incelenmiştir. PGT yapılan grupta FISH sonucu normal kromozom yapısına sahip embriyolar ile transfer uygulanmıştır. Bulgular: 23 PGT siklusunda biopsi yapılan 82 embryonun 69’unda floresan in situ hibridizasyon (FISH) sonrası sonuç alınmış (%84.1), analiz edilen 32 embriyoya ait blastomerede anomali saptanmıştır (%46.4). PGT sonrası kromozom yapısı normal olarak tespit edilen embriyolar ile uygulanan embriyo transfer işlemi sonrası 21 siklusta %33.3 gebelik oranı ve %25 implantasyon oranı elde edilmiştir. Abstract
Objective: The aim of this study is to evaluate the impact of PGD on implantation and ongoing pregnancy rates in couples whose male partner was diagnosed as having dominantly (>50%) spermatozoa with macrocephalic forms together with the other forms including multi-tailed sperm, sperm without tails and globozoospermia. Institution: İstanbul Memorial Hospital, ART, Reproductive Endocrinology and Genetics Centre, İstanbul Materials and Methods: 83 ART cycles of 73 couples in which male partner shows macrocephalic sperm characteristics were retrospectively analyzed in Istanbul Memorial Hospital, ART, Reproductive Endocrinology and Genetics Centre. In 23 cycles, suitable embryos were biopsied for p reimplantation genetic diagnosis (PGD) was performed on day 3 of embryo development. Blastomeres were then analyzed by fluorescent in situ hybridization (FISH) for chromosomes 13, 18, 21 Xand Y. In cases where PGD was perfomed, chromosomally normal embryos were selected for transfer procedure. Results: Out of 82 embryos biopsied in 23 PGD cycles, sixtynine of them gave interpretable results (84,1%) and abnormalities detected in 32 blastomeres analyzed (46,4%).
