Özet

Amaç: Tekrarlayan implantasyon yetersizliği olan ol­gularda düşük doz aspriin ve heparinin in-vitro fertilizasyon sonuçlarına etkileri Çalışmanın yapıldığı yer: Özel üremeye yardıma teda­vi merkezi Materyal ve Metod: IVF tedavi sırasında düşük doz as­pirin ve heparin uygulanmış hastalar ile aynı dönem içer­sinde ikinci kez siklus uygulanan fakat aspirin ve heparin kullanılmamış hastaların sonuçları retrospektif olarak araş­tırıldı. Bulgular: Her iki grup demografik karakterler olarak benzer saptandı, implantasyon oranları her iki grupda de-ğişmenıekle beraber (% 14.3 ile %14.4), kontrol grubundaki gebelik oranı anlamlı olarak fazla saptandı (%34.9 ile %52.7). Aspirin ve Heparin kullanılan grupta eve bebek gö­türme oranı %27.9'dur. Bu grupta gebe kalan hastaların %70'i 34. haftadan sonra canlı doğum yapmışlardır. Sonuç: Düşük doz aspirin ve heparin tedavisinin sonuş-ları tekrarlayan implantasyon yetmezliği olan hastalarda yüz güldürücüdür. Buna rağmen hasta seçim kriterleri klini­ğin tecrübesine göre değişmeli, tekrarlayan implantasyon yetmezliğinde embriyolardaki aneuploidi sıklığıda göz önünde bulundurulmalıdır.

Abstract

Aim: The impact of using low dose aspirin and heparin on the outcome of women undergoing IVF with history1 of recurrent implantation failure was assessed Institution: Private IVF center Materials and Methods: Women with the diagnosis of RIF undergoing IVF by using low dose aspirin and heparin were compared to controls as matched women undergoing their second IVF cycle during the same time period Results: Both groups were comparable in terms of ba­seline characteristics. Although implantation rates were not different (14.3% vs 14.4%, respectively), control group has significantly higher pregnancy rate (34.9% vs 52.7% res­pectively). Take home baby rate in asp+ hep group was %27.9 and almost 70% of women conceived among this group delivered after 34 weeks. Conclusion: The impact of using low dose aspirin and heparin among women with history of RIF was promising. However patient selection for this kind of treatment moda­lity should be also based on physician preference and ex­perience. On the other hand besides immunological factors, chromosomal aneuploidy should be kept in mind in RIF ca­ses.
