Özet
Amaç:Bu çalışmada vaden uzunluğunun normal olduğu atipik Rokitanski Küster Mayer Mauser (RKM) olgusu ile Marfan Sendromu birlikteliği tartışılmıştır. Çalışmanın yapıldığı yer: SSK Ankara Doğumevi ve Kadın Hastalıkları Eğitim ve Araştırma Hastanesi Olgu sunumu: 17 yaşında bekar hasta adet görememe şikayeti ile polikliniğimize başvurdu, Yapılan fizik muayenesinde büyüme ve gelişmesi normal olarak saptandı. Pelvik muayenede vagen derinliği normaldi. Pelvik ultrasonogra.fi-de uterus izlenmedi. Uterusun lokaüzasyonunda 19x8x10 mm boyutlarında ftbröz bir bant izlendi. Överler normal boyut ve görünümde idi. Karyotip 46.XX olarak rapor edildi. Hormon profili normogonadotropik olarak saptandı. Kemik mineral dansite ölçümünde osteoporoz tespit edildi. Bir yıl önce başka bir merkezde Marfan Sendromu tanısı almıştı. Ekokardiyografik incelemesinde mitral valv prolapsusu saptanmıştı. Bir yıl önce başka bir merkezde Marfan Sendromu tanısı almıştı. Ekokardiyografik incelemesinde mitral valv prolapsusu saptanmıştı. Sonuç: RKM olgularında IVP ile üriner sistem anormallikleri araştırılırken nadir görülse de kardiyak anomaliler açısından olgunun değerlendirilmesi gerekmektedir. (TürkFertil. Der. 2005; 13: 329-330)
Abstract
Objective: To report a case of an atypical Rokitansky-Küster-Hauser-Mayer (RKM) syndrome with normally developed vagina associated with Marfan's syndrome: Institution: Social Insurance Institution, Ankara Maternity and Women's Health Teaching Hospital, Department of Obstetrics and Gynecologoy, Ankara, Turkey Case: 17 years old, single patient applied to our outpatient clinic with the complaint of absence of menstruation. On the physical examination the growth and development of the patient were normal and the pelvic examination revealed the depth of vagina was normal. Uterus was absent but there was 19x8x10 mm fibrous band on the localization of the uterus at the pelvic ultrasonography. The size and appearance of both ovaries were normal. Karyotyping showed 46.XX. Hormonal status was within the normal limits. Bone mineral density measurement performed and osteoporosis detected. She had been previously diagnosed as Marfan's syndrome a year ago at another center and mitral valve prolapsus detected in this patient. Conclusion: Despite it is rarely seen, patients with RKM syndrome must be evaluated for cardiac abnormalities also, besides being investigated for urinary tract abnormalities by IVP.(Turkish J FertiL, 2005; 13: 329-330)
