Özet

Üremi kaşıntısı, böbrek hastalıklarının son dönemlerinde sık görülen ve yaşam kalitesini bozan semptomlardan biridir.Patogenezi hala yeterince aydınlatılamamış olduğundan tedavisi de güçtür ve çeşitli ilaçlar veya fiziksel yöntemler üzerinde çalışmalar sürmektedir. Serotoninin duyusal sinir uçlarındaki reseptörler aracılığıyla kaşıntı duyusunu uyardığı bilinmekte ve üremi kaşıntısında rol oynayabileceği düşünülmektedir. Bu açık, rastgele ve karşılaştırmalı çalışmada hemodializ uygulanan ve üremi kaşıntısı olan 20 hastada 5-hidroksitriptamin tip 3 (5-HT3) reseptör antagonisti olan ondansetron ile histamin 1(H1) reseptör blokeri olması yanısıra antiserotinerjik etki gösteren siproheptadinin etkinliği araştırılmıştır. On olguya 8 mg/gün ondansetron, diğer 10 olguya 8 mg/gün siproheptadin 30 gün süreyle verilmiştir. Abstract

Backround and Design: Uremic pruritus is a frequent symptom occuring in end-stage renal disease which interferes with the life quality of the patients. The pathogenesis of this symptom has not fully understood yet, so treatment is difficult and there are several studies on new therapeutic alternatives. Serotonin is known to induce pruritic symptoms through receptors on sensory nerve endings and it has been reported as a possible mediator of uremic pruritus. In this open, randomised and comparative study, the antipruritic effects of ondansetron which is a 5- hydroxytriptamine type 3 (5-HT3) antagonist and cyproheptadine which has both antihistaminic and antiserotinergic effects, were investigated in 20 hemodialysis patients. Material and Method:Ten patients were given 8 mg/day ondansetron and the other 10 patients were given 8 mg/day cyproheptadine orally for 30 days.
