İmmünoinhibisyon kreatin kinaz MB aktivite ölçümünün yanlış değerlendirilmesi: Tip I, Tip II makro CK ve CK BB

Özet
Akut miyokart infarktüsü (AMI) tanısında daha duyarlı ve daha erken tanı konulmasını sağlayan laboratuvar testleri olmasına rağmen, günümüzde en sık kullanılan laboratuvar testi immünoinhibisyon yöntemiyle kreatin kinaz MB (CK MB) izoenziminin ölçümüdür. Bu çalışmada AMİ şüphesi olan 269 hastaya fizik muayene, EKG ve tekrarlayan total CK, CK MB ölçümü yapıldı ve bu hastaların 27"sine AMİ tanısı konuldu. C K MB değeri yüksek ve total CK ile uyumsuz 65 vakaya CK elektroforezi yapıldı. Artmış CK MB ölçüm sonuçlarına göre değerlendirme yapıldığında 25 vakanın yanlış yüksek olduğu saptandı ve bunların 3 'ünde makro CK tip l (MCK-I) (%1.1), 2 'sinde makro CK tip U (MCK-II) (%0.7), 2 'sinde (%0.7) ise artmış CK BB tespit edildi. Artmış C K BB ve MCK-II vakalarının hepsinde, MCK-I li 2 vakada CK MB/total CK oranı 0.25 üzerinde iken; çeşitli kas hastalıkları bulunan veya operasyon geçiren toplam 6 vakada CK MB/total CK oranı 0,05 altında olduğu saptandı. Sonuçta, AMİ şüphesi ile yapılan MCK ve CK B.B insidansının normal popülasyonlara yakın oranlarda olduğu ve CK MB yüksekliği bulunan vakalarda CK MB/total CK oranını kullanılarak yanlış pozitif vakaların azaldığı, spesifitenin %89.7 den %95.0 a kadar yükseldiği saptandı.
Abstract
Although there are new biochemical tests for the diagnosis of acute myocardial infarction (AMI) which are more sensitive and earlier diagnostic markers, measurement of creatine kinase MB isoenzyme (CK MB) using immunoinhibition method is currently widely used. In this study, 27 of 269 patients with the prediagnosis of AMI who had under gone physical examination, electrocardiogram and repetitive measurements of total CK, C K MB were diagnosed as AMI. Electrophoresis of CK was applied to 65 cases whose C K MB values were increased and not correlated with total CK. Evaluation of CK MB values indicated that 25 cases were determined falsery positive and 3 of them had macro CK-I (MCK-I), 2 of them had macro CK-II (MCK-II) and 2 of them had increased CK BB. Though CK MB/total CK ratio was found higher than 0.25 in all of the cases with detected CK BB, MCK-II and 2 of the cases with MCK-l, this ratio was lower than 0.05 in all 6 cases with various muscular diseases. Consequently, it was determined that MCK and CK BB incidences in the AMI suspicion were similar to those in the normal population. It was also concluded that routine use of CK MB/total C K could give rise to a decrease in the falsery positive cases with elevated C K MB activities and to an increase in the specificity from 89. 7 % to 95 %
