Perkütan transluminal koroner anjioplasti sonrası restenoz gelişen hastalarda HDL ve HDL alt gruplarının analizi

Özet
Koroner Arter Hastalığı (KAH) tedavisinde yaygın bir şekilde kullanılan Perkütan Transluminal Koroner Anjiyoplasti (PTKA) ' nin uzun süreli etkinliğini sınırlayan majör problem restenoz gelişimidir. Günümüzde, birçok biyokimyasal parametrenin yanı sıra erken KAH gelişiminde bağımsız ve güçlü bir risk faktörü olan düşük düzeydeki Yüksek Yoğunluklu Lipoprotein Kolesterolü (HDL-K) ve Apolipoprotein AI (Apo AI)' in PTKA sonrası restenoz riski üzerine etkileri araştırılmaktadır. Çalışmamızda, PTKA sonrası restenoz gelişen 31 hasta (30 erkek ve 1 kadın) ve restenoz gelişmeyen 37 hasta (30 erkek ve 7 kadın) olmak üzere toplam 68 hastada HDL-K, HDL2-K, HDL3-K, Apo AI, total kolesterol, trigliserid, total/HDL-K düzeyleri çalışıldı. Burada HDL ve alt grupları ultrasantrifüj yöntemi ile elde edildi ve kolesterol düzeyleri çalışıldı. Çalışmamızda HDL-K, HDL2-K, HDL3-K ve Apo Al seviyeleri restenoz gelişen grupda sırasıyla 33±6, 15±4, 17±3 ve 113$pm$14 mg/dl iken restenoz gelişmeyen grupta sırasıyla 35±8, 16±5, 18±4 ve 122±18 mg/dl bulundu. Apo Al düzeyleri arasındaki fark anlamlı bulundu (p=0,036). HDL-K, HDL2-K, HDL3-K düzeyleri arasındaki fark anlamsız bulundu (sırasıyla p değerleri: 0,16, 0,26, 0,27). Restenoz gelişen hastaların 20' sinde (%65) HDL-K seviyeleri $leq$35 mg/dl iken 11 ' inde (%35) HDL-K seviyeleri >35 mg/dl bulundu (p=0,08). Abstract
The major problem that limits the long term -effectiveness of PTCA which is widely used in the Coronary Artery Disease (CAD) treatment is occurance of restenosis. At present, besides many biochemical parameters low HDL-C and Apo AI levels which is an independent and powerful risk factor to the early CAD occurance after PTCA restenosis risk is under investigation. In our study, we have investigated HDL-C, HDL2-C and HDL3-C levels in 68 patients. 31 patients who suffered from restenosis after PTCA (30 male and a female) and 37 patients who have no restenosis. The HDL and subgroups are researched with ultracentrifugation method and cholesterol levels are also studied. In our study, HDL-C, HDL2-C, HDL3-C and Apo AI levels of the first group ranked as follows: 33±6mg/dl, 15$pm$4 mg/dl, 17±3 mg/dl and 113±14 mg/dl while the levels of the second group whom didn' t have restenosis were as follows: 35$pm$8 mg/dl, 16±5 mg/dl, 18±4 mg/dl and 122$pm$18 mg/dl. In the statistical evaluation between these two groups, only the Apo AI level difference between two groups is found significant (p=0,036), differences on HDL-C, HDL2-C and HDL3-C levels are evaluated as nonsignificant (the p value of those were as follows: 0,16, 0,26 and 0,27). The HDL-C levels in 20 of 31 patients having restenosis (65%) were $leq$35 mg/dl while the HDL-C level in 11 patients (35%) were >35 mg/dl.
