Özet

Apoptozis, genetik olarak programlanmış morfolojik ve biyokimyasal değişiklikler ile karakterize hücre ölümüdür. Apoptozisin karsinogenez oluşumunda önemli rolü olduğu düşünülmektedir. Bunun yanısıra birçok antineoplastik tedavinin de etkilerini apoptozis aracılığıyla gösterdiği bilinmektedir. Bu çalışma , apoptozisin ve bazı onkogen, tümör süpresör genlerin mycosis fungoides patogenezindeki rollerini incelemek ve PUVA tedavisinin etki mekanizmasına açıklık getirebilmek amacı ile planlanmıştır. Mikozis fungoides tanısı alan 44 hastanın tedavi öncesinde ve 25 hastanın tedavi sonrasında c-myc, bcl-2, p53, PCNA ekspresyonları ve apoptozis araştırıldı. c-myc, bcl-2, p53, PCNA ekspresyonları immünuhistokimya yöntemi ile çalışıldı. Apoptozis parçalanmış DNA' nın in-situ olarak işaretlenmesi (in-situ end-labeling of fragmented DNA) yöntemi ile araştırıldı. çalışma grubundaki 44 hastanın %80'i Evre I'de, %9'u Evre II'de, %1'i Evre III'de idi. Lenfositlerdeki c-myc, bcl-2 ve PCNA ekspresyonları tedavi sonrasında anlamlı olarak düşme gösterdi. Tedavi öncesi ve sonrasına ait biyopsi örneklerinde nadir apoptotik hücre saptandı. Sayının yetersiz olması nedeniyle değerlendirmeye alınamadı. Çalışmamız PUVA tedavisinin Mycosis fungoides lezyonlarında, lenfositlerdeki c-myc, PCNA ve bcl-2 ekspresyonlarını azalttığını göstermekte ve lenfositlerde hem proliferasyonu engellediği, hem de apoptozise neden olduğu görüşünü desteklemektedir.

Abstract

Background and design: Apoptosis and oncogenes, tumor suppressor genes have prominent role in the homeostasis between cell proliferation and death. This may be an important mechanism in the pathogenesis of mycosis fungoides which especially in early stages is claimed to be disease of lymphoaccumulation rather than lymphoproliferation. Our purpose was to bring a better understanding to this suggested mechanism. Materials and methods: We studied expressions of c-myc, bcl-2, p53, PCNA and apoptosis in lesional skin biopsies of 44 patients with mycosis fungoides in different stages (Stage I: %80, Stage II: %9, StageIII:%11) before treatment and after treatment in 25 patients. Expressions of c-myc, bcl-2, p53, PCNA were evaluated by immunohistochemistry. In-situ end-labeling of fragmented DNA technique was used for detection of apoptotic cells. Results: Expressions of c-myc, bcl-2 and PCNA in lymphocytes significantly decreased after treatment with PUVA. Pre and post treatment biopsy samples failed to show any significant apoptotic cells. Conclusion: Our study shows that PUVA treatment decreases expressions of c-myc, bcl-2 and PCNA expressions in lymphocytes. It inhibits lymphocyte proliferation and seems to induce apoptosis.
