Verruka tedavisinde 5-fluorourasil'in intralezyonel kullanımı: Plasebo kontrollü tek kör çalışma

Özet

Verruka tedavisinde lokal olarak kullanılan 5-fluorourasili (5-FU) İntralezyonel uygulayarak, etkinliğini arttırmayı amaçladık. Aynı hastada hem 5-FU hem de plasebo uygulayabilmek için çalışmaya birden fazla lezyonu (ortalama 3,1 lezyon, aralık:2-6 ) olan 86 hasta (47 kadın 39 erkek;yaş ortalaması 23,4 aralık: 16-48 yaş) alındı. Hastalar randomize olarak iki gruba ayrıldı. Birinci gruba sadece 5-FU ve plasebo (serum fizyolojik) uygulanırken, ikinci grupa lidokain+adrenalin/5-FU karışımı (1/5 oranında) ile plasebo uygulandı, injeksiyonlar 1cc/cm2 dozda ve haftada 1 sıklıkta, toplam 4 kez; kontroller son uygulamadan 1 ve 2 ay sonra kör olarak yapıldı. Uygulamalardaki ağrı skorlandı ve sonrası bul, nekroz, sikatris ve pigmentasyon gelişimi kaydedildi. Hastalarda tedavi boyunca ve kontrollerde sistemik şikayet gelişmedi ve rutin tetkiklerde değişiklik olmadı. 5-FU uygulanan 45 hastada, 26 tam, 13 kısmi yanıt görüldü ve 6 hasta yanıtsız kaldı. Lidokain+adrenalin/5-FU uygulanan 41 hastanın 25'inde tam, 9'unda kısmi iyileşme gözlenirken, 7 hastada ise tedaviye yanıt alınamadı. Özellikle mozaik verrukalarda birinci grupta 6/6 ikinci grupta 2/3 hastada tam düzelmeyle, yüksek etkinlik tespit ettik istatistiksel değerlendirme chi kare testi ile yapıldı. Her iki grup plaseboya karşın etkisi anlamlı olmakla beraber (p0,05). Sonuç olarak verruka tedavisinde 5-FU İntralezyonel kullanımı, yaşam kalitesini bozmadan uygulanabilecek etkin bir yöntemdir.

Abstract

Background and design: There are many treatment modalities for the therapy of common warts. The cytocytatic drug 5-FU has been found to selectively affect squamous cell, but the method of its delivery directly into affected tissue such as warts has been of limited efficacy. This study is designed to assess the efficacy of intralesional administration of 5-FU. Materials and methods: Eightysix patients (47 female, 39 male; mean age 23.4 years, range 16-48) who had warts more than one were entered into study (mean number of lesions: 3.1; range 2-6) . Thus placebo and therapeutic agent could be applied to the same patient. Each lesion was injected once a week for up to 4 weeks, and patients were evaluated at first and second months after the last injection. Pain and cutaneous reactions were noted. The patients were randomized into two groups; 5-FU with placebo and lidocaine+adrenalin/5-FU with placebo. Results: Of the 45 patients who applied 5-FU, 26 went into full remission, 13 partially and 6 not responded. In second group (lidocaine+adrenalin/5-FU), of the 41 patients, 25 went into full remission, 9 partially and 7 not responded. Especially mosaic verrucae were well responded to the therapy. Statistical analyses was performed with chi-square test. Both 5-FU and lidocaine+ adrenalin/5-FU treatments were effective: both were superior to placebo (p 0.05). Scarring and pigmentary changes at injection sites were minimal. No systemic side effects were observed. Conclusion: Intralesional administration of 5-FU with or without lidocaine proved to be an effective method in more than half of the patients with verrucae.
