Özet
Watershed infarktlar (WSİ) ön, orta ve arka serebral arterlerin sulama alanlarının sınır bölgelerinde görülen enfarktlardır. Tek taraflı watershed infaktlar, çoğunlukla önceden var olan ipsilateral vasküler hastalık zemininde sistemik hipotansiyon episodu sırasında oluşan fokal serebral hipoperfüzyona bağlı olarak gelişmektedir. Bu çalışmada ardışık olarak seçilen 156 iskemik strok hastasından bilgisayarlı tomografi görüntülerinde watershed infarkt saptanan 16 olgu dahil edildi (%10.6). Ön WSİ 4 olguda (%25), arka WSİ 5 olguda (%31) ve orta WSİ 7 olguda (%44) saptandı. Olguların 8'inde orta ve ciddi derecede karotis stenozu, 2'sinde karotis tıkanıklığı saptandı. Altı hasta hipotansiyona meyilli idi. Ortostatik hipotansiyon 4 olguda saptandı. WSİ'ların patogenezinden başlıca hemodinamik fenomen sorumlu tutulmaktadır. Hipotansiyona eğilimli, ciddi karotis stenozu olan, birlikte kalp hastalığı bulunan risk grubunda yeterli kan basıncı ve serebral perfüzyonun sağlanması hemodinamik yetmezlikten ve serebral iskemiden korunmak için önemlidir.
Abstract
Watershed infarcts (WSİ) can be defined as the kind of infarcts observed within the borderzones of anterior, middle and posterior cerebral arteries in watershed territories. The unilateral WSİs often develop due to focal cerebral hypoperfusion which often occurs during the systemic hypotension episode in the preexistent base of ipsilateral vascular pathology, in this study, 16 cases of ischaemic stroke, that are determined to have WSİs in computerised tomography among 156 ischaemic stroke patients who were elected ordinarily. Anterior WSİ was determined in 4 cases (25%), posterior WSI was determined 5 cases (%31) and internal WSİ was determined 7 cases (%44). Among these cases, eight of them had medium and severe carotid stenosis and 2 of them carotid occlusion. Six of the patients were found to be inclined to hypotension. Orthostatic hypotension was determined in 4 cases. Chiefly hemodynamic phenomenon is believed to be responsible for the pathogenesis of WSİ. We have, in consequence, suggested that, in order to be protected from hemodynamic failure and cerebral ischeamie, it would be of crucial importance to provide cerebral perfusion and effective blood pressure in the group inclined to hypotension and carrying cardiac failure associated with severe carotid stenosis.
