Özet
HELLP Sendromu hemoliz, karaciğer fonksiyon testlerinde yükselme ve trombositopeni ile karakterize, gebelik hipertansiyonunun bir komplikasyonu olan klinik bir tablodur. Bu olguda; HELLP sendromu tanısı konduktan sonra fetal distress nedeniyle acil operasyona alınarak düşük doz hiperbarik bupivakain ile spinal anestezi uygulanan 35 haftalık preeklamptik bir gebe sunulacaktır. Bölgesel anestezi anne ve fetus için bir avantaj teşkil etmekle birlikte, koagülopati bu yöntem için bir risk oluşturmaktadır. Bu olgu sunumunda belirtildiği gibi, koagülopatisi bulunmayan HELLP sendromlu hastaların sezaryen operasyonlarında spinal anestezi uygun bir yöntem olabilir.
Abstract
HELLP Syndrome is characterized with hemolysis, elevated liver enzyme levels, thrombositopenia and it is a complication of hypertension of pregnancy. In this case report, we present a preeclamptic woman with a 35- week gestation who underwent emergency cesarean section because of fetal distress, following the diagnosis of HELLP syndrome. The cesarean section was performed under spinal anaesthesia with low dose of hyperbaric bupivacaine without any complication. Regional anaesthesia offers advantages for mother and fetus but presence of coagulopaty is a risk factor for this technique. Providing normal coagulation profile, spinal anaesthesia may be an appropriate method for cesarean section in patients with HELLP syndrome, as demonstrated in this report.
