İzole laringeal havayolunda normoksik ve hiperkapnik gaz akımının solunum faaliyetleri üzerindeki etkileri

Özet

Larinkste lokalize olan mekanosensitif sonlanmaların hava akımı ile uyarılmaları, refleks olarak ventilasyonu azaltmaktadırlar. Ayrıca bu reseptörler % 5 CO2-hava karşımına da duyarlıdırlar. Bilindiği gibi hiperkapni solunum üzerinde uyarıcı etkiye sahiptir. Çalışmamızda laringeal mekanoreseptörlerin iki farklı konsantrasyonda hiperkapni ile uyarılmalarının soluk hacmi ve frekansına olan etkilerini inceledik. Deneyler Na-tiyopenton ile anestezi uygulanan (25 mg kg-1) tavşanlarda yapıldı. Larinks oropharyngeal boşluktan ve trakeadan ayrıldı. Spontane hava soluyan deney hayvanlarında solunum pompası ile normoksik ve hiperkapnik gaz (% 7 CO2-% 93 hava) ve (%12 CO2-hava) karışımlarının laringeal hava yolundan geçirilmesinde aşağı trakeal kanül aracılığı ile soluk hacmi (VT), soluk frekansı (f/dk) poligrafta kaydedildi ve solunum dakika hacmi (VE) hesaplandı. Ayrıca, sistemik arter kan basıncı kaydedildi. Her bir fazda alınan arteriyel kan örneklerinde PaO2, PaCO2 ve pH tayin edildi. Deneysel olarak izole edilmiş laringeal hava yolundan pompa aracılığı ile dakikada 500 mL akım hızında ve dakikada 73 frekansında ortam havası geçirildiğinde, soluk hacminde önce bir azalma sonra normale dönüş gözlendi. Aynı hızda % 7 CO2-hava ve % 12 CO2-hava gaz karışımları geçirildiğinde, soluk hacminde ve frekansında daha fazla azalma gözlendi. VT'deki azalma CO2 konsantrasyonu ile orantılı idi. Bilateral orta servikal vagotomiden sonra larinksten hiperkapnik gaz karışımları geçirildiğinde VT'de yine anlamlı azalma gözlenirken frekansta vagotomi öncesi gözlenen cevap ortadan kalktı. Vagotomiden sonra larinksin afferent innervasyonunu sağlayan superior laringeal sinir bilateral olarak kesildiğinde, her iki hiperkapnik faz karışımına cevabın tamamen ortadan kalktığı gözlendi (% 0.4±0.4+% 0.3±0.5). Sonuç olarak bulgularımız, laringeal mekanoreseptörlerin hava akımına olduğu gibi hiperkapniye de duyar olduğunu ve CO2'in hava akımı ile oluşan solunumsal inhibisyonu kuvvetlendirdiğini göstermektedir.

Abstract

The effects of air flow rate and CO2 on the isolated laryngeal airway and the resultant changes of ventilation. The stimulation of the mechanosencitive endings localised in the larynx with air flow decreases the ventilation by reflex pathway. Moreover, these receptors were shown to be sensitive to airway hypercapnia. The purpose of this study was to investigate the effects of the stimulation of mechanoreceptors on ventilatory parameters. For this purpose two different concentrations of carbon dioxide were used. The experiments were carried out in anesthetised (thiopentone 25 mg kg-1) IV spontaneously air breathing rabbits. The larynx was seperated from the oropharyngeal cavity and from the trachea. Tidal volume (VT), respiratory frequency f (min-1) were recorded from lower tracheal cannula below the separation. Respiratory minute volume (VE) was calculated. Systemic arterial blood pressure was recorded. At the end of each experimental phase systemic PaO2, PaCO2 and pHa were determined. An initial decrease observed in VT and f/min on passing of the room air at a flow rate of 500 mL min-1 at a frequency of 73 min-1 from the experimentally isolated laryngeal airway by means of respiratory pump.The VT and f (min-1) values reverted to the control value during the following breaths. On passing of 7 % CO2 and 12 % CO2 at the same flow rate a greater decrease than of the air following phase was observed in VT and f (min-1). The response of VT was proportinate with the concentration of CO2. Following bilateral cervical vagatomy, although there was no change in f (min-1) in response to 7 % CO2 and 12 % CO2; VT was again found to decrease significantly. After cutting the superior laryngeal nerve in addition to vagotomy, the responses of VT to both hypercapnic gas mixtures were abolished (0.4 %±0.4+ 0.3 %±0.5). In conclusion, the results of this study suggest that the laryngeal mechanoreceptors are sensitive to air flow rate and hypercapnia. The respiratory inhibition produced by increased air flow is enhanced by concentration of CO2.
