Özet
Cerrahi ameliyat esnasında meydana gelecek kanamanın önlenmesi gerek cerrah gerekse de anestezist için önemlidir. Çalışmamızın amacı kontrollü hipotansiyon amacıyla kullanılan magnezyumun kanama miktarı, cerrah kanama skoru, koagülasyon testleri ve hemodinamik parametreler üzerine etkilerini tesbit etmektir. Bu çalışma, septorinoplasti yapılan ASA I fiziksel skora sahip 40 erişkin olgu üzerinde gerçekleştirildi. Olgular 50 mL serum fizyolojik içinde 50 mg kg-1 magnezyum sülfat bolus ve 15 mg kg-1 sa-1 infüzyon verilen Magnezyum (n=20) ya da 50 mL serum fizyolojik bolus ve infüzyon verilen Kontrol (n=20) gruplarına ayrıldı. Ameliyat süresince hemodinamik parametreler 5 dakikada bir kaydedildi. Tüm olgulardan ameliyat öncesi, ameliyat sonrası ve ameliyattan 6 saat sonra serum magnezyum, kalsiyum, protrombin zamanı, parsiyel tromboplastin zamanı, uluslararası normalizasyon olanı, fibrinojen miktarı, trombosit sayısı ve hemoglobin düzeyleri çalışmak üzere venöz kan örnekleri alındı. Abstract
The prevention of bleeding that will occur during surgical procedure is very important for the surgeon and for the anaesthesiologist. The aim of the current study is to determine the effects of magnesium that can be used in controlled hypotension on amount of bleeding, surgeon bleeding score, coagulation tests and hemodynamic parameters. This study was performed in fourty ASA physical score I adult patients who underwent septorhinoplasty procedures. Patients were divided as Magnesium (n=20) and Control groups (n=20). The patients in the magnesium group were given a bolus dose of magnesium sulp-hate 50 mg kg-1 in 50 mL of serum physiologic solution and had magnesium infusion at a rate of 15 mg kg-1 h-1 whereas the patients in the control group received a bolus of 50 mL of serum physiologic solution and infusion. During the procedure hemodynamic parameters were recorded in every 5 minutes. Venous blood samp-les were taken for analysing serum magnesium, calcium, hemoglobin and fibrinogen levels, prothrombin time, partial thromboplastine time, INR, and platelet count before and after the operation and at the postoperative 6th hour. In Magnesium group, the difference of serum fibrinogen level between before operation value and after operation value was lower than the difference of serum fibrinogen level between before operation value and after operation value in placebo group (p0.05).
