Özet

Psödokolinesteraz, lipid ve lipoprotein metabolizmasında rol alan, süksinilkolin ve mivakuryumun yıkımını sağlayan önemli bir enzimdir. Bu makalede preoperatif tanı almamış psödokolinesteraz eksikliğine bağlı nöromüsküler (NM) blok uzamasıyla birlikte hiperglisemi gelişen 2 olgu sunuldu. Ameliyat tamamlandıktan sonra spontan solunumu dönmeyen bu hastalarda Train-of-Four (TOF) stimulasyonuna cevabın olmadığı gözlendi. Ayrıca geçici hiperglisemi geliştiği saptandı. Uzamış NM blok olgularında psödokolinesteraz eksikliğine bağlı gelişen hiperglisemi, psödokolinesteraz ile glukoz arasındaki korelasyon literatürle birlikte tartışıldı.

Abstract

Pseudocholinesterase is an enzyme functioning in the metabolism of lipids and lipoproteins and ıt is important in the degradation of succinylcholine and mivacurium. In this report, we presented two cases who had hyperglycemia with concomitant prolongation of the neuromuscular block due to pseudocholinesterase deficiency that was not diagnosed prior to operation. At the end of the operation, the patients had no return of spontaneous respiration and response to TOF stimulation. In addition, there was temporary hyperglycemia. Hyperglycemia due to pseudocholinesterase deficiency, and relation between pseudocholinesterase and glucose were discussed in prolonged neuromuscular block cases according to the literature.
