Ehlers Danlos sendromlu bir gebenin anestezik yöntemi

Özet
Ehlers Danlos Sendromu anormal prokollajen ve kollajen üretimi ile karakterize bir grup herediter bağ dokusu hastalığıdır. Bu olgu sunumunun amacı Ehlers Danlos Sendromu olan bir gebenin anestezik yönetimini tanımlamaktır. 33 yaşında ve 33. gebelik haftasında olan ve Ehlers Danlos Sendromu olan gebe sezaryen için ameliyata alındı. Olguda dört yıl önce genel anestezi altında sezaryen sırasında gelişen kardiyak arrest ve kardiyopulmoner resusitasyon öyküsü mevcut. Hastanın ilk sezaryen ameliyat sırasında kardiyak arrest ve kardiyopulmoner resusitasyon öyküsü mevcuttur. Hastada aynı zamanda iki yıl önceki laparaskopik kolesistektomi ameliyatı sırasında pnömotorax gelişmiştir. Sezaryen için alınan olgunun preoperatif klinik muayenesi, ekokardiyografi ve koagülasyon faktörleri (PT, aPTT, Trombosit sayımı) değerlendirildi. Tüm bıulgular normaldi. Hasta daha önceki genel anestezi deneyimlerinde birkaç ciddi komplikasyon geçirdiği için biz spinal anesteziyi tercih ettik. Anestezi öncesi 500 mL Ringer Lactat solüsyonu infüze edildi. Hasta sol yan pozisyonda iken 27 gauge spinal iğne L4-5 interspinöz aralığa ilerletildi. Spinal iğnenin doğru yerleştirilmesinden sonra 20 mg ropivacaine spinal aralığa uygulandı. Hemodinamik ve solunumsal parametreler kaydedildi ve ameliyat süresince herhangi bir komplikasyon ile karşılaşılmadı. Biri erkek diğeri kız sağlıklı ikiz yenidoğan doğurtuldu. Sonuç olarak, Ehlers Danlos Sendromlu gebelerde spinal anestezi etkili ve anne ve bebek açısından mükemmel sonuçlarla birliktedir.
Abstract
Ehlers Danlos Syndrome is a group of inherited connective tissue disorders which are characterized by the production of abnormal procollagen and collagen. The aim of this case report is to describe the anaesthetic management of a parturient with Ehlers Danlos syndrome (EDS). A 33-yr-old pregnant woman with EDS at 33rd weeks of gestation was admitted for cesarean section. She had a previous history of cardiac arrest and cardiopulmonary resuscitation during cesarean section under general anaesthesia four years ago. Also, the patient had experienced pneumothorax during laparoscopic cholecystectomy two years ago. After admission for cesarean section, preoperative clinical examination, echocardiography and coagulation studies (prothrombin time, activated partial thrombin time, platelet count) were evaluated. All findings were normal. We preferred spinal anaesthesia for cesarean section because she had exposed to several complications during previous general anaesthesia experiences. 500 mL Ringer Lactate was infused before anesthesia. With the patient in the lateral decubitis position, a 27 G spinal needle was inserted into the L4-5 intervertebral space. After correct placement of the spinal needle, ropivacaine 20 mg was administered. Hemodynamic and respiratory parameters were noted and no problem was met during the procedure. Healthy infant twins, one male and the other female, were delivered. In conclusion, spinal anaesthesia was effective and associated with excellent maternal and fetal outcomes in pregnancy with EDS.
