Özet
Bu çalışmada; günübirlik artroskopi uygulanacak olgularda, sabit doz propofol ile farklı infüzyon hızlarında uygulanan remifentanil kombinasyonunun derlenme kriterleri üzerine etkilerinin mental ve psikomotor fonksiyonlar yönünden değerlendirilmesi amaçlanmıştır.Çalışmaya artroskopi planlanmış, ASA I-II grubundan 90 erişkin olgu dahil edildi. Anestezi uygulaması öncesinde değerlendirilen mental (VAS) ve psikomotor fonksiyonlar "digit symbol substitution test" (DSST) kontrol değerler olarak kabul edildi. Tüm olgularda propofol (2 mg kg-1) ve remifentanil (1 µg kg-1 dk-1) ile anestezi indüksiyonu gerçekleştirildi. Endotrakeal entübasyonu gerçekleştirmek amacıyla cis-atrakuryum 0.15 mg kg-1 uygulandı. Anestezi uygulaması 4-5 mg kg-1 sa-1 propofol ve remifentanil infüzyonu ile sağlandı. Olgular rasgele üç gruba ayrıldı; remifentanil infüzyonu, grup I'de 0.20 mg kg-1 dk-1, grup II'de 0.25 mg kg-1 dk-1 ve grup III'de ise 0.30 mg kg-1 dk-1 olarak uygulandı. Anestezi sonrası olguların göz açma, emirlere uyma, oryantasyon, oturma ve yürüme zamanları ile taburcu olma zamanları kaydedildi. VAS ve DSST değerleri postoperatif 1., 5., 15., 30., 45., ve 60. dakikalarda izlendi ve not edildi.Grupların demografik verileri ve anestezi süreleri arasında istatistiksel olarak anlamlı bir farklılık saptanmadı. Göz açma, komutlara uyma, oryantasyon zamanı grup I'de diğer gruplarla kıyaslandığında daha kısa olarak saptandı. Abstract
The aim of this clinical investigation was to compare the recovery times and cognitive function in patients undergoing outpatient arthroscopy, after different remifentanil doses in combination with a hypnotic dose of propofol.Ninety adult ASA class I-II patients were included into the study. Prior to anesthesia, visual analog scala (VAS) patients were given baseline assessments of mental state (VAS) and cognitive function "digit symbol substitution test" (DSST). Anaesthesia was induced with propofol 2 mg kg-1 and remifentanil 1 µg kg-1. To perform endotracheal intubation cis-atracurium 0,15 mg kg-1 was given.Anaesthesia was maintained with propofol 4-5 mg kg-1 h-1 and remifentanil infusion at three rates as 0.20 µg kg-1 min-1, 0.25 µg kg-1 min-1, and 0.3 µg kg-1 min-1 in three groups randomly. VAS and DSTT values were recorded at 1, 5, 15, 30, 45 and 60 min postoperatively. The times to be able to sit, walk, open eyes, respond to command and discharge were also recorded.Demographic variables and duration of anaesthesia were similar in groups. Significantly shorter times to be able to sit, walk, open eyes, respond to command and discharge were observed in group I when compared with the other groups.
