Özet

Deneysel çalışan kalp modelinde, miyokardiyal iskemi öncesi,sonrası ve reperfüzyon sonrası dönemlerde, tiroid hormonları ve kortizol düzeylerindeki değişiklikler araştırıldı. 10 Mongrel köpek (20-25 kg) ketamin (5 mg kg-1) ve ksilazin (2 mg kg-1) ile sedatize edildi. Anestezi indüksiyonunda tiyopental-Na (7 mg kg-1) ve fentanil (1 mg kg-1), idamede ise isofluran (% 1-1.5) ile fentanil (1 mg kg-1) ve vekuronyum kullanıldı. Kalp sol torakotomi ve perikardın vertikal açılımı ile eksplore edildi. Sol anterior koroner arter Octopus® 2 stabilizatör kullanılarak stabilize edildi ve 15 dakika "buldog" klemp ile oklüze edilerek iskemi sağlandı. Hemodinamik parametreler torakotomi öncesi ve sonrası, iskemi öncesi, iskemi döneminin sonunda ve 10 dakikalık reperfüzyondan sonra kaydedildi. Aynı dönemlerde, plazma tiroid stimulan hormon, total tiroksin, serbest tiroksin, total triiodotironin, serbest triiodotironin ve kortizol düzeylerinin değerlendirilmesi için kan örnekleri alındı. Abstract

The alterations in thyroid hormones and cortisol levels were investigated before ischaemia and after the myocardial ischaemia and after myocardial reperfusion periods in an experimental model for beating heart. Ten Mongrel dogs (20-25 kg) were sedated with ketamine (5 mg kg-1) and xylazine (2 mg kg-1). Anesthesia was induced with thiopental sodium (7 mg kg-1) and fentanyl (1 mg kg-1), and was maintained with isoflurane (1-1.5%), fentanyl (1 mg kg-1), and vecuronium. The heart was exposed via left thoracotomy and additional vertical dissection of the pericardium. The left anterior coronary artery was stabilized using Octopus® 2 and occluded with a "bulldog" clamp for 15 minutes. Hemodynamic parameters were recorded before and after thoracotomy, before ischaemia (occlusion), at the end of the ischemic period, and after 10 minutes reperfusion. Blood samples were taken to measure of plasma thyroid-stimulating hormone, total thyroxine, free thyroxine, total triiodothyronine, free triiodothyronine and cortisol hormones at the same time intervals. Additionally, transmural myocardial biopsy specimens were taken with "Tru-cut" needle before ischaemia and after reperfusion.
