Azotprotoksitin zamana göre endotrakeal kaf basıncına etkisi

Özet

Çalışmamızda; azotprotoksit+oksijen ile uygulanan genel anestezi sırasında endotrakeal kaf basıncı artışının zamana göre değerlendirilmesi amaçlandı. Genel anestezi altında baş boyun cerrahisi ve laparoskopik cerrahi dışında supin pozisyonda opere olacak ASA I-III grubunda 60 olgu çalışmaya dahil edildi. Tüm olgulara; 2 mg kg-1 propofol ve 2 µg kg-1 fentanil ile anestezi indüksiyonu, 0.6 mg kg-1 rokuronyum ile kas gevşemesi sağlandı ve polivinil klorürden yapılmış, yüksek volüm, düşük basınçlı, kaflı uygun çapta endotrakeal tüp ile entübasyon yapıldı. Endotrakeal tüp kafı hava ile, kaçak sesi olmayacak şekilde şişirildi ve basınç manometresine bağlandı. Başlangıçtaki kaf basınçları kaydedildi. Olgular rasgele 2 gruba ayrıldı. Grup A (Azotprotoksit grubu) olgulara % 60 N2O+% 40 O2+% 2-2.5 sevofluran, Grup H (Hava grubu) % 50 hava+% 50O2+1 µ kg-1 sa-1 fentanil infüzyonu+% 2-2.5 sevofluran uygulandı. Başlangıç kaf basıncı 35 cmH20'dan fazla ise kaf basıncı kaçak olmayacak şekilde 25-30 cmH2O basıncına indirildi. Kaf basıncı entübasyondan sonraki 5-10-15-30-45-60-75-90-120. dk.'larda kaydedildi. Kaf basıncı, 45 cmH20 basıncınca ulaştığında ölçüm zamanı beklenmeksizin başlangıç değerine indirildi. Postoperatif derlenme odasında ve postoperatif 1.gün boğaz ağrısı olup olmadığı soruldu. Abstract

In the current study we aimed to assess endotracheal tube cuff pressure changes in time when using nitrous oxide+oxygen. Sixty adult patients, who underwent elective surgery with general anaesthesia in supine position, ASA physical statuses I-III were included in the study. Following the induction, patients were intubated with a suitable size, high volume, low-pressure polyvinyl chloride tube. Endotracheal tube cuffs were inflated until the audible gas leak was just eliminated and monitorized with a pressure manometer. Initial cuff pressure were recorded and decreased to 25-30 cm H2O if the initial recordings were above 35 cm H2O. Patients were divided into two groups. The patients received 60 % N2O+40 % O2+2-2.5 % sevoflurane in group A (nitrous oxide group) and 50 % air+50 % O2+2-2.5 % sevoflurane with 1 µ kg-1 h-1 fentanyl infusion in group H (air group). Cuff pressures were recorded at 5, 10, 15, 30, 45, 60, 75, 90, 120 minutes after intubation. Cuff pressure was decreased to initial measurements immediately whenever it exceeded 45 cm H2O. Patients were questioned about sore throat in postoperative recovery room and at the first day of the operation. The increases in cuff pressures were significant in group A and the cuff pressures exceeded 45 cmH2O after 45 minutes. In group H pressures were significantly lower than initial measurements after 60 minutes. As a result we concluded that cuff pressures may rise to the degrees which can totally impede mucosal blood flow in 45 minutes when using nitrous oxide in general anesthesia.
