Özet

Magnezyum sülfat, motor sinir ucundan asetil kolin salınımını önleyerek, nondepolarizan nöromuskuler ilaçların etkisini potansiyelize etmektedir. Bu çalışmada, magnezyum sülfatın rokuronyum bloğu üzerine etkisini araştırmak amaçlandı. Etik Kurul onayı alındıktan sonra, ASA I-II grubu 40 erişkin olgu çalışmaya dahil edildi. Tüm olgular operasyondan 30 dk. önce midazolam (0.05 mg kg-1; im) ile premedike edildi. EKG, pulsoksimetre, noninvaziv kan basıncı, kapnografi, ısı probu ve ulnar sinir trasesi üzerinden TOF Guard ile monitorizasyon gerçekleştirildi. TOF-Guard twitch (tw) kalibrasyon işlemi yapıldı. Olgular rasgele 20'şer kişilik iki gruba ayrılarak indüksiyon öncesi Grup I'e magnezyum sülfat (60 mg kg-1; iv) % 0.9 NaCl ile 100 mL volüme tamamlanarak, Grup II'ye ise 100 mL % 0.9 NaCl 15 dk sürede infüze edildi. Anestezi indüksiyonu, fentanil (1 µg kg-1; iv) ve tiyopental sodyum (5 mg kg-1; iv) ile yapıldı. Rokuronyum (0.6 mg kg-1; iv) 5 sn'de verildi. Tw % 0 süresi saptanarak entübasyon gerçekleştirildi ve kali-tesi değerlendirildi. Tw % 10 ve % 25 iyileşme süreleri ve derlenme indeksi (Tw % 25-75) kaydedildi. Abstract

Magnesium sulphate inhibits the release of acetylcho-line from the motor nerve terminal and thus potentiates the action of the non depolarizing neuromuscular blocking drugs. The aim of this study is to investigate the effects of magnesium sulphate on rocuronium-induced neuromuscular block. After obtaining institutional Ethics Committee approval, ASA I or II, 40 patients were stutied. All patients were premedicated with midazolam (0.05 mg kg-1; im) 30 minutes before the operation. The patients were monitored with ECG, pulse oxymetry, noninvasive blood pressure cuff, capnography, thermal probe and TOF-Guard response. TOF-Guard twitch calibration process was done. The patients were randomly seperated into two groups. Patients in Group I, MgSO4 (60 mg kg-1; iv) in saline, total amount of 100 mL was infused as an iv infusion over 15 min. Patients in group II the same volume of saline was infused without MgSO4. In both groups, anaesthesia was induced with fentanyl (1 µg kg-1; iv) and thiopentone (5 mg kg-1; iv) . Rocuronium (0.6 mg kg-1; iv) was given in 5 sec. Tw 0 % time was detected and intubation was done and quality of intubation was assessed. Tw 10 % and 25 % recovery time and recovery index were noted. The study groups did not differ in age, sex and weight. Time of 0 % Tw was significantly shorter in Group I (p
