İntravenöz hasta kontrollü analjezide morfine bağlı bulantı ve kusmada "Reliefband" ve metoklopramidin etkinliği

Özet
Çalışmamızın amacı, majör ortopedik cerrahilerde, postoperatif hasta kontrollü analjezide kullanılan morfinin neden olduğu bulantı ve kusmada, ReliefBandın etkinliğini değerlendirmektir.18-70 yaşlarında 42 hasta çalışmaya alınarak, randomize olarak 3 gruba ayrıldı. İndüksiyondan 15 dk. önce antiemetik yöntem olarak, Reliefband grubuna (Grup RB) sağ önkol P6 akupunktur noktasına aktif Reliefband (25mA, 4Hz) ile günde 3 defa 2 cc iv salin, metoklopramid grubuna (Grup M) günde 3 defa iv 10 mg metoklopramid ile inaktif Reliefband, kontrol grubuna (Grup K) inaktif Reliefband ile günde 3 defa 2 cc iv salin uygulandı. Tüm gruplarda indüksiyon 5-7 mg kg-1 tiyopental sodyum, 1 µg kg-1 fentanil, 0.1 mg kg-1 veküronyum bromid, idame % 50 O2 + % 50 N2O ve % 1.5 izofluran ile sağlandı. Tüm dönemlerde RB ile M grupları arasında bulantı ve kusma yönünden anlamlı fark yokken, bulantı sayısı kontrol grubuna göre daha azdı (p0.05). Abstract
The purpose of our study to compare the effect of Reliefband applied to the P6 acupuncture point, with metoclopramide on nausea-vomiting related to the postoperative morphine use by patient controlled analgesia in major orthopedic surgery. 42 patients between the ages of 18-70, were randomly allocated into 3 groups. İn group Reliefband (GroupRB), active RB (25mA, 4Hz) to the P6 acupuncture point of the right forearm and 2 cc iv saline 3 times daily; in group metoclopramide (Group M) inactive RB and iv l0 mg metoclopramide 3 times daily; in group control (Group C), inactive RB and 2 cc iv saline was given 15 minutes before induction. In all groups induction was performed with thiopental sodium 5-7 mg kg-1, 1 µg kg-1 fentanyl and vecuronium bromide 0.1 mg kg-1. Anaesthesia was maintained with 1.5 % isoflurane in 50 % O2 and 50 % N2O with a flow rate of 6L.There were no significant differences between the group RB and group M in terms of nausea-vomiting but the incidence of nausea in both groups was less than that of group C (p0.05).
