Özet

Amaç: Bu çalışmada aort cerrahisi sırasında ,derin hipotermik sirkülatuar arrest (DHSA) uygulanan bir seri olguyu sunmayı amaçladık.Gereç ve Yöntem: Şubat 2001 ile Şubat 2004 yılları arasında DHSA ile aort cerrahisi yapılan 12'si acil ve 22'si elektif 34 olgu retrospektif olarak değerlendirildi.Bulgular: Kardiyopulmoner bypass'a (KPB) girildikten sonra nazofarinks ısısı 15±0.3°C de DHSA'ya girildi. Olgulardan 31 (% 91)'ine retrograd serebral perfüzyon (RSP) yapıldı. DHSA süresi 40 dk.'yı geçen 3 olguda antegrad serebral perfüzyon (ASP) uygulandı. Cerrahi işlem bitiminde 36°C'de KPB sonlandırıldı. Erken mortalitenin % 14 olduğu görüldü. Olgulardan biri dirençli hipotansiyon diğeri gaz degişimindeki bozukluk nedeniyle KPB den ayrılamadı. Yoğun bakımda ise 3 olgu renal yetmezlikten kaybedildi.Yoğun bakımda ortalama ekstübasyon süresi 31±4.8 saat, entübasyon süresi 48 saati geçen olgu (% 17)) ve yoğun bakımda kalış süresi 4.5±0.8 gün olarak saptandı. Bir olguda geçici stroke olurken kalıcı nörolojik sekel hiçbir olguda görülmedi. Olguların 5'inde (% 15) plevral effüzyon saptandı. Altı olguda (% 18) tedavi gerektiren aritmi saptanırken birine kalıcı pacemaker takıldı. Hastanede kalış süresi ortalama 9±0.7 gündü.Sonuç: İncelediğimiz bu seride DHSA 40 dakikadan az sürdüğünde RSP ile DHSA 40 dakikadan fazla sürdüğünde ASP ile nörolojik defisit gözlemedik.

Abstract

Aim: We present, a case series of, patients in whom deep hypothermic circulatory arrest (DHCA) was used during aortic surgery.Materials and Methods: Thirty-four patients undergoing DHCA for aortic surgery (emergent 12, elective 22) between February 2001 and February 2004 were retrospectively rewiewed.Results: Mean nasopharyngeal temperature was 15.0±0.3°C during cardio pulmonary bypass (CPB) when DHCA was initiated. Retrograde cerebral perfusion (RCP) with DHCA was applied in 31 (% 91) patients. The other 3 patients had longer duration of DHCA (>40 minutes) and received antegrade cerebral perfusion (ACP). At the end of surgery CPB was terminated when nasopharyngeal temperature was> 36°C.Early mortality was 14 %. Two patients were unable to be weaned from CPB one due to persistant hypotension and the other due to impaired gas exchange. Three patients, all requiring renal replacement therapy,died in the ICU. In survivors , mean extubation time and ICU length of stay were 31±4.8 hours and 4.5±0.8 days respectively six patientsr (17 %) were intubated > 48 hours. Transient neurological deficit were observed in one patient. Permanent neurological deficit was not observed in any survivor. Pleural effusion was seen in the 5 (% 15) patients. Arrhythmia requiring treatment was observed in 6 (% 18) patients. Permanent pacemaker was used in one patient. Mean hospital length of stay was 9±0.7 days.Conclusion: In this case series we observed no permanent neurologic deficit fallowing with RCP when DHCA was 40 minutes
