Özet

Amaç: Bu çalışmada orta kulak cerrahisine yönelik hipotansif anestezide remifentanil-sevofluran ve remifentanil-propofol kombinasyonlarının perioperatuvar kanama, hemodinami ve postoperatuvar derlenme üzerine etkilerinin karşılaştırılması amaçlanmıştır.Gereç ve Yöntem: ASA I-II grubu 36 olgu rastgele 2 gruba ayrıldı. Anestezi indüksiyonunda, 1 µ kg-1 remifentanil 0.6 mg kg-1 roküronyum, Grup S'de % 6 sevofluran, Grup P'de 2 mg kg-1 propofol uygulandı. Anestezi idamesinde 0.25 µ kg-1 dk-1 remifentanil infüzyonu, Grup S'de % 1-2 sevofluran, Grup P'de 6 mg kg-1 saat-1 propofol infüzyonu ile O2-N2O karışımı kullanıldı. Olgular sistolik, diastolik, ortalama arter basınçları (SAB, DAB, OAB), kalp atım hızı (KAH), tahmini kan kaybı derlenme kriterleri bakımından değerlendirildi.Bulgular: Her iki gurupta da OAB ve KAH anestezi döneminde induksiyon öncesi değerine göre düşüktü. Gruplar arasında hemodinamik açıdan farklılık yoktu. Her iki gurupta da MAP induksiyon öncesi değerlere göre ekstübasyondan sonra yüksekti, bununla beraber guruplar arasında fark yoktu. Abstract

Aim: The aim of this study was to compare remifentanil-sevoflurane and remifentanil-propofol hypotensive anaesthesia, for in middle ear surgery and associated, perioperative bleeding, haemodynamics and postoperative recovery characteristics.Materials and Methods: Thirty-six ASA physical status I-II patients were randomized into two groups. Induction of anaesthesia consisted of remifentanil (1 µg kg-1), rocuronium (0.6 mg kg-1) and either, 6 % sevoflurane (Group S) or 2 mg kg-1 propofol (Group P) Maintenance of anaesthesia consisted of a continuous infusion of remifentanil 0.25 µg kg-1 min-1, and 1-2 % sevoflurane (Group S) or 6 mg kg-1 h-1 propofol continuous infusion (Group P) with O2-N2O. Systolic, diastolic and mean arterial pressures (SAP, DAP, MAP), heart rate (HR), estimated blood loss and recovery characteristics were evaluated.Results: MAP and HR were lower during anesthesia compared to pre-induction values in both groups. There were no difference in haemodynamic variables between groups.
