Düşük akımlı desfluran anestezisinde klonidinin titreme üzerine etkisi

Özet
Amaç: Isıtılmayan cerrahi hastalarda hipotermi gelişir ve bu da operasyon sonrası termoregülatuvar vazokonstriksiyon (VK) ve titremeyi tetikler. Klonidin, hipotalamusta bulunan alfa-2 adrenerjik reseptörler aracılığıyla VK ve titreme için eşik değeri düşürür. Çalışmamızda, düşük akımlı desfluran anestezisi sonrası postoperatif titreme oranını ve klonidinin titreme üzerine etkisini incelemeyi amaçladık.Gereç ve Yöntem: Etik kurul onayı alındıktan sonra, 18-60 yaş arası, ASA I-II, 40 hasta çalışmaya alındı. Her iki grupta anestezi indüksiyonu iv bolus propofol (2 mg kg-1) ve fentanil (1 µg kg-1) ile, idame ise desfluran (% 5-6) ile sağlandı. Taze gaz akımı ilk 10 dk. 6 L dk-1 (O2 /N2O: 2/4), daha sonra 1 L dk-1 (O2/N2O: 0.5/0.5) olarak uygulandı. Anestezi sonlandırılmadan 10 dk. önce tekrar yüksek akıma geçildi. Rasgele iki gruba ayrılan hastalara 10 mL içerisinde iv 2 µg kg-1 klonidin (Grup I, n=20) veya serum fizyolojik (Grup II, n=20) verildi. Kalp atım hızı (KAH), ortalama arter basınçları (OAB), rektal ısı değerleri kaydedildi. Postoperatif titreme "3 noktalı skala" ile değerlendirildi.Bulgular: İntraoperatif rektal ısıdaki azalma gruplar arasında farklı bulunmadı (Gr I: 0.66±0.32°C, Gr II: 0.90±0.46°C) (p=0.06). Klonidin grubunda anlamlı olmasa da daha az oranda titreme gözlendi (klonidin: % 20, kontrol: % 45). Titremenin şiddeti ve süresi açısından gruplar arasında fark yoktu.İntravenöz 2 µg kg-1 klonidin'in operasyon sonunda verilimi ile ciddi yan etki gözlemeksizin postoperatif titreme oranını kontrol grubuna göre anlamlı olmasa da düşük tespit ettik.Sonuç: Klonidin grubundaki titreme oranının daha düşük olmasını, klonidin'in VK ve titreme için eşik değeri düşürmesiyle açıklayabiliriz.
Abstract
Aim: Unwarmed surgical patients may become hypothermic after an operation. A decrease ın core temperature wıll trigger thermoregulatory vasoconstriction and shivering. Clonidine lowers the threshold for cutaneous vasoconstriction and shivering via hypothalamic alpha-2 adrenoreceptors. In our study, we aimed to investigate the incidence of postoperative shivering and the effects of clonidine on shivering after low flow desflurane anaesthesia.Materials and Methods: After approval from the ethics committee, we included 40 patients (18-60 year olds, ASA I-II). Anaesthesia was induced with an iv bolus of propofol (2 mg kg-1) and fentanyl (1 µg k-1) and was maintained with desflurane (5-6 %) in both groups. A fresh gas flow of 6 L min-1 (O2/N2O: 2/4) was given during the first 10 minutes, then reduced to 1 L min-1 (O2/N2O: 0.5/0.5). The patients were randomly assigned to receive iv 2 µg kg-1 clonidine (Group I, n=20) or saline (Group II, n=20). Heart rate, mean arterial pressure, and rectal temperature values were recorded. Postoperative shivering was assessed by using 3-point scale.Results: There was no difference between the two groups with respect to the decrease of rectal temperature in the intraoperative period (Gr I: 0.66±0.32ºC, Gr II: 0.9±0.46ºC). Postoperative shivering was observed less in the clonidine group than the saline group (clonidine: 20 %, saline: 45 %) (P>0.05). There was no difference between the two groups with respect to the severity and duration of shivering.We have demonstrated that clonidine (2 µg kg-1) given late in the intraoperative period, reduces the incidence of postanaesthetic shivering without any increase in side effects as compared with the control group with no statistically significant difference in shivering.Conclusion: We believe, the lower shivering rate in the clonidine group can be explained by the threshold lowering effect of clonidine on vasoconstriction and shivering.
