Özet

Amaç: Genel anestezi uygulanan hastalarda, cerrahiden hemen ve yirmi dört saat sonrasına ait endişelerinin neler olduğunu araştırmayı amaçladık.Gereç ve Yöntem: Etik kurul izni ile onay alındıktan sonra 173 olgu çalışmaya alındı. Standart soru anketinde ağrı, bulantı, kusma, çevreyi tanımama, bilinçsizlik hali, titreme, boğaz ağrısı, uykuya meyilli olma, tüpü hissetme, susuzluk hissi, ameliyatı hatırlama semptomlarının puanlama yapılarak değerlendirilmesi istendi. Puanlama görsel nümerik değerlendirme tekniği ile yapıldı ve her bir semptoma; 1 puan (en az endişe duyulan semptom) ile 10 puan, (en çok endişe duyulan semptom) arasında puan verilmesi istendi.Bulgular: Olgular (4.6±3.6) puan vererek en çok ameliyat sonrası ağrı duyma endişesini taşıdıklarını belirttiler. Operasyonun birinci günü ise, ağrı puanı (5.8±3.7) artış gösterdi. Bilinçsizlik hali (3.2±4.0), boğaz ağrısı (2.8±3.44), çevreyi tanımama (2.5±3.5) ve uykuya meyil (2.1±2.9) puan aldı. Ağrı ile ilgili endişe operasyon sonrası birinci günü belirgin olarak artarken diğer faktörlerde ameliyat öncesinde verilen puanlara göre azalma gösterdiler.Sonuç: Genel anestezi alan hastalarda, ameliyat sonrası ağrı, en yoğun endişe kaynağıydı; bu bulgular, ameliyat sonrası dönemdeki ağrı kontrolünün operasyon öncesinde hasta ile detaylı olarak konuşulmasının önemini ortaya koymuştur

Abstract

Aim: We aimed to determine factors causing anxiety immediately and 24 hours after surgery in patients undergoing general anaesthesia.Material and Methods: Following the Institutional Ethics Committee approval and informed consent, 173 cases were studied.A standard evaluation form rated patient anxiety about postoperative pain, nausea, vomiting, disorientation, shivering, sore throat, drowsiness, thirst, gagging on the tracheal tube and awareness during anaesthesia. Anxiety was rated using a 1 to 10 point verbal "numeric anxiety scale". (NAS) where a score of "1" represented "least upsetting condition" and "10" represented the "most upsetting condition".Results: Pain caused the most anxiety (immediately postoperative NAS=4.6±3.6 increasing to 5.8±3.7 on postoperative day one. Awareness during anaesthesia (3.2±4.0), sore throat (2.8±3.4), disorientation (2.5±3.5) and drowsiness (2.1±2.9) were other major causes of anxiety. While the increase in anxiety related to pain increased significantly on postoperative day one, there was a decreased in changes in the other factors over this time.Conclusion: Postoperative pain is the most common anaesthesia-related factor causing anxiety in patients undergoing general anaesthesia. This finding underscores the importance of detailed preoperative communication with patients regarding their perioperative pain management.
